2. A 42 yr old patient was admitted in hospital on 14-4-14. c/o : Joint pain Tremors Tiredness.

3. Past medical history : Not a known case of DM/ HTN Past medication history : Nill

4. Social history : Non-Alcoholic Non-smoker Known allergies - Nill Marital status – Married General examination :- Consicous Oriented

5. P°/I◦/ cl °/cy°/ ly °/e°. Local examination : Temp. : 98.4° B.P : 130/90 mm Hg Pulse : 75 beats/min

6. CVS : S1 S2 + R.S : B/L Air entry P/A : Soft. CNS : NAD

7. Lab investigation : All lab investigations are normal. Other investigation : Bone mineral density – 1.9 g/cm3 ** {Normal – 2.5g/cm3}

8. Diagnosis : OSTEOPOROSIS { porous bone }

9. Drug chart : S.no Drugs R.o.A Dosage 1. Inj. Visconix { NA HYALURONATE } I.v 20mg + _ _ 2. Inj. Voveran { Diclofenac } I.V 5cc + + + 12 th 13 th 14 th

10. SOAP NOTES : S – SUBJECT 42 yrs old female patient was admitted in hospital complaining ; Joint pain, Tremors, Tiredness.

11. O – OBJECTIVE ; Temp., B.P, Pulse are normal A – ASSESSEMENT ; With the help of other investigation and o/e we can diagnose that she has OSTEOPOROSIS.

12. P – TRAETMENT PLAN ; Administration of Inj. Visconix & Inj. Voveron controls osteoporosis.
