Introduction to Acute Infections DEFINATION : Rapid-onset, painful infections of the periodontium (gingiva, PDL, bone). KEY TYPES  Gingival Abscess  Periodontal Abscess  Pericoronitis  Necrotizing Periodontal Disease [NPD]

The New Coronavirus Pathogenesis MECHANISM:  Bacterial invasion [e.g., P. gingivalis, Fusobacterium spp.]  Biofilm disruption → Host immune overresponse → Pus formation TRIGGERS:  Trauma, foreign bodies, poor oral hygiene

The New Coronavirus The New Coronavirus 1. Gingival Abscess FEATURES:  Localized to marginal/papillary gingiva  Pain, erythema, fluctuant swelling MANAGEMENT:  Incision/drainage + remove foreign body

2. Periodontal Abscess DEFINATION : Pus in a pre-existing periodontal pocket. RISK FACTOR : Untreated periodontitis, diabetes, smoking. TREATMENT : Drainage + scaling/root planing + antibiotics (amoxicillin + metronidazole).

3.Pericoronitis DEFINATION: SYMPTOMS: MANAGEMENT:  Inflammation around partially erupted teeth (e.g., wisdom teeth) Pain, trismus, halitosis . Irrigation (chlorhexidine) → Extraction if recurrent

The New Coronavirus 4. Necrotizing Periodontal Disease (NPD) Adults Seniors SIGNS: RISK GROUPS: TREATMENT: Punched-out papillae, pseudomembrane, bleeding. Immunocompromised (HIV, malnutrition) Debridement + metronidazole + nutritional counseling

The New Coronavirus Diagnosis 04 CLINICAL EXAM: RADIOGRAPHS: CARRANAZA’S TIP: Probing depth, suppuration, mobility Vertical bone loss patterns . Differentiate from periapical abscess (vitality test!)

The New Coronavirus Differential Diagnosis 2 3 RULE OUT: Periapical abscess (endodontic origin) Herpetic gingivostomatitis (viral vesicles). Angular cheilitis.

The New Coronavirus Emergency Management Protocol STEPS: Drainage (incision or through pocket) Antibiotics (if systemic symptoms: amoxicillin 500mg TID x 5 days) Definitive treatment (SRP, surgery, extraction) .

The New Coronavirus CASE STUDY PATIENT: DIAGNOSIS: TREATMENT: 40-year-old male with diabetes, sudden pain in lower left molar . Periodontal abscess (probing depth: 9mm, suppuration). Drainage + systemic antibiotics + referral for periodontal surgery
