AUMENTO DA FREQUÊNCIA EM GESTANTES Alguns grupos populacionais são mais susceptíveis a complicações e evolução para as formas mais graves da dengue, entre eles as gestantes e puérperas , especialmente até 14 dias pós-parto , em decorrência do retorno lento das alterações fisiológicas aos padrões pré-gestacionais .  INTRODUÇÃO

Dados  preliminares  ( ainda   não   divulgados ) do Sistema de  Informação  de  Agravos  de  Notificação / Coordenação -Geral de  Vigilância  de  Arboviroses  (SINAN/CGARB/ MS)  indicam  que o  número  de  casos  de dengue  em   gestantes   aumentou   consideravelmente   em  2024.  INTRODUÇÃO

Dados  preliminares  ( ainda   não   divulgados ) do Sistema de  Informação  de  Agravos  de  Notificação / Coordenação -Geral de  Vigilância  de  Arboviroses  (SINAN/CGARB/ MS)  indicam  que o  número  de  casos  de dengue  em   gestantes   aumentou   consideravelmente   em  2024.  INTRODUÇÃO

  Os arbovírus são organizados filogeneticamente em seis famílias ( Togaviridae , Flaviviridae, Bunyaviridae , Reoviridae, Rhabdoviridae e Orthomyxoviridae). O DENV pertence à família Flaviviridae, tal informaão é importante no momento do diagnóstico   diferencial   clínico e laboratorial. O DENV, por   apresentar elevada taxa de mutação  (RNA), possui quatro sorotipos diferentes , denominados DENV-1, DENV-2, DENV-3 e DENV-4. O DENV

Estudos apontam  que as taxas de óbitos maternos por dengue entre 2007-2015 foi  4 vezes maior entre gestantes com dengue, com pior prognóstico  no 3 trimestre ; O seguinte manual visa incluir no grupo de risco , tanto as gestantes quanto as puérperas até 14 dias de pós-parto , devido as modificações do organismo materno durante este período . Embora a infecção pelo DENV esteja associada a uma doença autolimitada , na maioria dos indivíduos ela apresentará sintomas leves da doença , estimando -se que menos de 5% evoluirão para quadros mais graves. Fases clínicas : febril , crítica e de recuperação . Fisiopatologia

Após um período inicial de incubação de tipicamente três a sete dias , ela se manifesta com início súbito de febre acompanhada de viremia elevada , conhecida como fase febril .  Alguns indivíduos passam para a fase crítica , a qual está associada ao extravasamento de plasma, enquanto outros prosseguem diretamente para a fase de recuperação , sem desenvolver extravasamento plasmático . Fisiopatologia

A  fisiopatologia da doença em gestantes e puérperas não difere daquela da mulher não grávida , mas a hemodiluição fisiológica da gravidez poderá mascarar a trombocitopenia , a leucopenia e a hemoconcentração associadas à dengue; Por sua vez , alguns dos sinais e sintomas da doença poderão ser confundidos , como náuseas , vômitos , dor abdominal, hipotensão postural e taquicardia , retardando o diagnóstico e as medidas de hidratação precoce , e predispondo à evolução de maior gravidade . Fisiopatologia

Clínico : sinais e sintomas ; sinais de alerta  para gravidade . Na fase inicial , as manifestações clínicas são dominadas por febre , cefaleia , exantema maculopapular, mal- estar , astenia , mialgia , artralgia ( normalmente de baixa intensidade ) e dor retro- orbitária . Complicações que devem alertar o obstetra para uma evolução mais grave:  dor abdominal intensa , vômitos , derrames das cavidades virtuais , hipotensão , hepatomegalia , manifestações hemorrágicas e sinais de comprometimento do sistema nervoso central ( letargia e/ ou irritabilidade ).  Diagnóstico

Laboratorial O hemograma traz informações importantes como a agressão indireta à medula por ação viral, com redução dos elementos figurados do sangue (notadamente a plaquetopenia) e hematócrito elevado decorrente do extravasamento líquido que ocorre em resposta ao aumento da permeabilidade capilar .  OBS: Sempre ser notificado no SINAN.
