Patient details Chanda Devi 81 years /Female Residence:Sirsa ,Haryana CrNo:939152401485667 Presentation to OPD:2/12/24 Diagnosis: Gall stone induced acute Necrotising pancreatitis with CTSI Score :8/10,/ARDS/HYPOCALCEMIA/HYPERTENSION Presented to OPD with C/o Pain in the abdomen for 1 day history which was sudden in onset,progressive in nature ,since for 24 hours,pain was in the epigastric and hypochondrial region,radiating to back,aggrevated by taking fatty meals ,relieved by Medications H/o vomiting present 2 days back,non projectile,non bilious,2 episodes ,relieved by medications O/E Patients Gc fair No pallor,pedal edema Afebrile BP:139/75mmhg PR:108/min SPO2:98 percent with RA P/ A:soft ,mild distensiom,Tenderness in the epigastric region and the umbilical region,BS + RS:bilateral air entry present and equal No added sounds Other system examination normal Plan of management :Conservative management Date of Death:24/12/24
