Definition: Buergers disease is a rare disease of the arteries and veins in the arms and legs. Blood vessels become inflamed, swell and can become blocked with blood clots( thrombi). This eventually damages or distroy skin tissues and lead to infection and gangrene Its usually first shows in your hands and feets and may affect larger areas of arm and legs.
Pathophysiology: The specific cause of Buerger’s disease remains unknown. The risk for developing Buerger’s disease increases when you smoke heavily . Scientists don’t know why tobacco smoke increases this risk, but the correlation between the two is well documented. According to the researches, almost everyone with Buerger’s disease uses tobacco.
Risk factors: Tobacco smoking Male gender Rickettsial infection South Asian and middle eastern Age between 22 to 45 years Medical history of Raynaud's and autoimmune disease.
Symptoms: On and off pain in your legs, feet,arm and hand. Pain increase with movement and decrease with rest. Inflammation along a vein just below the skin surface . Fingers and toes turn pale when exposed to cold. Painful open sores in fingers and toes.

The Allen's test Your doctor may perform a simple test called the Allen's test to check blood flow through the arteries carrying blood to your hands. In the Allen's test, you make a tight fist, which forces the blood out of your hand. Your doctor presses on the arteries at each side of your wrist to slow the flow of blood back into your hand, making your hand lose its normal color. Next, you open your hand and your doctor releases the pressure on one artery, then the other. How quickly the color returns to your hand may give a general indication about the health of your arteries. Slow blood flow into your hand may indicate a problem, such as Buerger's disease.
Treatment: MEDICINE TREATMENT Smoking cessation Medicines to dilate the blood vessels and improve circulation (prostaglandins, prostacyclin) Intermittent compression of arms and legs to increase blood flow.. Surgery in sever cases Amputation in infection and gangrene.

BUERGERS ALLENS EX or Ratschow's Ex Lower extremities are elevated 45 to 90 degrees angles until skin blanches account.(2to 3 mints) Feets and legs are then lower than rest of the body unless redness appears.(5 to 10 mints) Finally legs are placed flat on the bed for few minutes.(for 10 mints)
