Chisturile ovariene sunt relativ frecvent intalnite la femeile de varsta fertila. Multe femei au chisturi ovariene la un moment dat in timpul vietii, majoritatea sunt de mici dimensiuni si dispar de la sine.
Uneori chisturile ovariene pota avea o structura complexa, cot contine componente solide, ridicandu-se suspiciunea de laignitate. Chisturile maligne sunt apar mai ales la femeile cu varsta peste 40 de ani.
Cauze
Cauzele chisturilor ovariene depind de varsta pacientei.
Fetite. La fetele care nu au ajuns inca la pubertate, chisturile de mici dimensiuni sunt comune. Cand chisturile au dimensiuni mari, pot fi un semn al pubertatii precoce.
Adolescente. La fetele la care menstruatia s-a instalat, chistul este, de regula, functional (simplu), legat de ciclul menstrual, si se resoarbe fara tratament.
Varsta adulta – cele mai multe chisturi ovariene se dezvolta ca rezultat al activitatii ciclului menstrual (chisturi functionale).
Simptome
Majoritatea chisturilor ovariene functionale sunt de obicei inofensive si nu produc simptome. Dispar fara tratament si pot fi descoperite in timpul unei examinari pelvine de rutina.
Cu cat chistul ovarian este mai mare, cu atat este mai probabil ca el sa produca simptome. Unele chisturi pot determina senzatie de presiune, durere surda abdominala sau senzatie de greutate abdominala. Durerea poate aparea in timpul actului sexual (dispareunie). Unele chisturi produc hormoni care afecteaza perioadele menstruale – ciclul menstrual poate deveni neregulat sau cu sangerare mai abundenta. La femeile in postmenopauza, aceste chisturi pot cauza sangerarea vaginala. De asemenea pot apare greturi, varsaturi.
Cand chistul se rupe sau devine strangulat, pacienta resimte la nivelul abdomenului o durere ascutita, severa, insotita de greata si febra, o durere care se manifesta brusc. Poate apare frbra, senzatia de frig, vertij, slabiciune, accelerarea respiratiei. Atunci cand apar aceste manifestari, femeia trebuie sa mearga la medicul ginecolog.
Chisturi ovariene functionale- tipul cel mai intalnit. Adesea se remit spontan. Chisturile ovariene functionale sunt clasificate in:
Chisturile foliculare - sunt cele mai frecvente. Ovulul, celula sexuala feminina, se dezvolta la nivelul ovarului intr-o structura numita folicul. Foliculul contine un lichid care protejeaza ovul si care este eliminat in momentul ovulatiei. Daca nu se produce ruptura folicului si eliberarea ovulului, folicului creste in dimensiuni si se umple cu lichid – chist folicular. De cele mai multe ori, chistul dispare fara tratament in cateva saptamani.
Chisturile luteinice - sunt mai rare si se dezvolta in tesutul restant in urma ovulatiei (corp galben sau corpus luteum). Frecvent, se remit spontan in cateva luni, dar uneori se pot rupe, ducand la sangerare interna si durere brusca.
Chisturi patologice
Chistul dermoid. Poate contine o varietate de tesuturi cum ar fi: par, tesut epitelial, dinti etc. Necesita interventie chirurgicala.
