. CARIPILL carica papaya leaf extract By: Dr. Sunny Kumar Gupta Clinical Pharmacologist, HCG Cancer Hospital, Bangalore Pharm.D (Doctor Of Pharmacy)

. General description: Caripill is a carica papaya (CP) extract. It is a member of the Caricaceae and is a dicotyledonous, polygamous, and diploid species. It originated from Southern Mexico, Central America, and the northern part of South America. It is now cultivated in many tropical countries such as Bangladesh, India, Indonesia, Sri Lanka, the Philippines, West Indies and Malaysia . Carica papaya is a rich source of complex biochemical constituents including flavonoids, glycosides, alkaloids, glutathione & glucosinolates The alkaloids, flavonoids, saponins , tannin, and glycosides are related with anti- inflammatory activity. CP leaves extract also found to have anti-bacterial effect, anti tumor, and immunomodulator activities (increase the production of signaling molecules such as T helper 1- type cytokines) . The leaf of CP is categorized as non toxic because its LD50 > 15 g per kg body weight. The leaves also contain cardiac glycosides, anthraquinones , carpaine , pseudocarpaine , phenolic compounds.

. In addition to the nutritional value of its fruit, the leaves of CP possess medicinal properties and are widely used in traditional medicines. Previous studies on papaya have shown that seed extract of CP possess pharmacological activities, including anthelminthic, antifertility, contraceptive etc. A hot-water extract of the leaves is taken orally as an antipyretic, and treatment of anemia, appetite stimulation. In many countries the leaves extract of CP had been effectively used for treatment of dengue fever (DF) associated with thrombocytopenia . Caripill is a formulation of Tablets & Syrup containing papaya (Carica papaya) leaf extract. Each Caripill tablet contains 1100 mg of Carica papaya leaf extract and Caripill Syrup contains 275 mg / 5 ml Carica papaya leaf extract The extract in Caripill is found to be useful in increasing the platelet count. This medicinal use of Caripill is proven through clinical studies in patients in India & other parts of the world with low platelet count associated with dengue.

. The drug Caripill launched by micro labs, Bengaluru, and the clearance was received from Department of Ayush . The principal investigator Dr A C G owda has conducted Clinical trial among 300 patients , showed promise results without the need of blood transfusions .

. Pharmacological Action: Platelet Booster :- Carica papaya is a rich source of complex biochemical constituents including flavonoids, glycosides, alkaloids, glutathione & glucosinolates that helps boost the number of platelets by stimulating bone marrow. C. papaya increases ALOX-12 & PTAFR gene expression by 15 & 13.42 folds that further boost the platelet count by increasing megakaryocytic production. It can also prevent platelet destruction in the blood and thereby increase the life of the platelet in circulation . Immunomodulator :- Carica papaya extracts show immunomodulatory activity by increasing the production of signaling molecules such as T helper 1- type cytokines & bone marrow cells significantly indicating stimulatory effect on entire haemopoetic system.

. Indications:- Thrombocytopenia due to Dengue Malaria, Typhoid fever Immunomodulator Digestion Platelet booster, Improves Blood Clotting Acute microbial infection Chemotherapy Dosage:- One tablet of Caripill (1100mg ) should be taken three times a day for 5 days. For children more than 1 year and less than 5 years: 275mg (5ml) three times a day for 5 days. For children more than 5 years and less than 18 years: 550mg (10ml) three times a day for 5 days .

Pharmacokinetics:- Absorption: well absorbed, increases effectiveness with food Bioavaibility: 50-60% Metabolism: Liver Half life: 6 hour Excretion: Urine & stools Price: Rs375 per 10 Tablets

. Contra-indications :-   Hypersensitivity, Pregnancy. In males with prostate dysfunction, such as BPH or prostate cancer, C. papaya should be avoided as it increases the iron absorption. Excess iron may increase oxidative stress, especially in the aging male. Iron overload may increase the risk of developing prostate cancer. Precautions and Interactions:   - Should be used with caution in individuals with bleeding disorders or those taking blood thinning medications such as aspirin or warfarin. Co administration of extracts of Carica papaya with oral hypoglycemic may lead to very low blood glucose as observed in one of the experimental animal study. Thus it is important to closely monitor the blood glucose levels regularly to avoid hypoglycemia. It has been found to increase the bioavailability of amiodarone and therefore the dose should be adjusted accordingly when coadministered with CP leaf extract .

. In vitro study demonstrated potentiating the action of various antibiotics like penicillin G, ampicillin, amoxyclav , cephalothin , polymyxin B, rifampicin, amikacin , nalidixic acid, gentamycin, cholarmphenicol , oflxacin when co administered with CP. The extract of C. papaya with antimicrobial agents possesses synergistic properties which act against the pathogenic organisms . Delays the onset of hypoglycemic activity of glimepiride , and increases the hypoglycemic effect of metformin. Adverse effects:  Nausea &Vomiting, Drowsiness Abdominal pain, Heartburn, Dyspepsia, Carotenemia Yellowing of soles and palms At high doses may cause stomach irritation

. Thank You!!
