2. Sociodemographic details A 65 year old hindu male patient named Punabhai Gamara,resident of Jilariya , Tq.Paddhari , Dist.Rajkot ,farmer by occupation presented to the AIIMS OPD with the chief complaint of diffused right upper arm redness and swelling with pain and tenderness

3. History of present illness The patient was apparently alright 8 days ago & got insect bite while driving the animals in forest, 6 days back he had swelling and redness with pain and tenderness over the affected area , 4 days back He took medication and dressing From the private hospital Yesterday he presented to the AIIMS OPD and then got admitted to surgical IPD for the debridement for cellulitis which involves anterior of right forearm There’s no history of bleeding or any fluid at the time of bite

4. Past medical history No any significant history No history of diabetes, thyroid bradycardia was present

5. Past surgical history History of appendectomy 20 years ago No any other significant history

6. Personal history Vegetarian Bladder normal Bowel : constipation detected No any addiction

7. Vitals BP : 120 /70 Temperature: 97.1°F PR: 52/ min Breathing rate: 18/ min No sign of pallor icterus Cynosis clubbing lymphadenopathy
