INTRODUCTION Rehabilitation is care that can help you get back, keep, or improve abilities that you need for daily life . These abilities may be physical, mental, and/or cognitive (thinking and learning). 

Cont… Community - based rehabilitation  (CBR) is community action to ensure that people with disabilities have the same rights and opportunities as all other  community members. This includes, for example, equal access to health care, education, skills training, employment, family life, social mobility and political empowerment.

DEFINITION CBR is implemented through the combined efforts of people with disabilities themselves, their families, organizations and communities, and the relevant governmental and non governmental health, education, vocational, social and other services"

GOAL The key activities of the CBR programme include: training family and community members on disability and CBR using the WHO CBR training manual as a guide; referring people with disabilities to specialist services, e.g. surgical and rehabilitation services, where physiotherapists, speech therapists and occupational therapists are available; providing assistive devices , e.g. walking sticks, crutches, wheelchairs, hearing aids, glasses;

Cont… creating employment opportunities by providing access to training, job coaching and financial support for income generation activities; providing support for social activities including for sports and recreation;

THE ROLE OF CBR The role of CBR is to promote, support and implement rehabilitation activities at the community level and facilitate referrals to access more specialized rehabilitation services

MAJOR OBJECTIVES To ensure that PWDs (person with disabilities) are able to maximize their physical and mental abilities , To access regular services and opportunities, and to become active contributors to the community and society at large . To activate communities to promote and protect the human rights of PWDs through changes within the community, for example, by removing barriers to community , to participation .

ASPECTS OF CBR TO STRENGTHENED THE PROGRAMMES The participants identified the following aspects of CBR that need to be strengthened to make programmes more effective: Community Involvement and Ownership Multi sectoral Collaboration in CBR programmes Involvement of Disabled People’s Organizations in CBR programmes Scaling up CBR programmes CBR programmes to be used as an effective tool for poverty reduction Evidenced based practice to promote CBR programmes

THE MATRIX The matrix illustrates the topic areas which can make up a CBR strategy. It consists of five components, each divided into five elements. Each of these elements will have a dedicated chapter in the guidelines. The elements are sub-divided into content headings.

MULTI-SECTORAL SUPPORT FOR CBR Support from the Social Sector : Examples: disability pensions, technical aids and adaptations, vocational training and employment, and co-ordination of referrals for individuals who require services from other sectors Support from NGOs and the local community Most communities have a variety of non-governmental organizations (NGOs) and groups that can contribute to a CBR programme .

Cont… Support from the Health Sector : The Declaration of Alma Ata (1978) states that Primary Health Care (PHC) is the key to attaining health for all . It also states that the PHC needs to address the main health problems in the community providing promotive preventive, curative and rehabilitative services .

Cont… PHC can play a major role in this context both as a provider and supporter: Early identification of impairments and providing basic interventions. Referrals to specialized services such as physical, occupational and speech therapies; prosthetics and orthotics; and corrective surgeries..

Cont… Support from the Educational Sector: Adapting the content of the curriculum and methods of teaching. Ensuring that classrooms, facilities and educational materials are accessible. Children with multiple or severe disabilities may access education through the use of innovative methods best suited to their context.

Cont… Support from the Employment and Labour Sector: Collaboration with the employment and labour sectors. Productive and decent work in a conducive environment Equal employment opportunities through national policies and legislation;

Cont… Support from the Media : All CBR stakeholders should work closely with the media to identify priorities and to provide relevant information.

STEPS OF CBR Identification of person requiring rehabilitation services. Assessment of disabilities and various needs for rehabilitation of identified person. Provide the basic services through PHC, such as drugs, dressing, materials, protective foot wear, counseling and training in self care. Introduce / escort the person to village health committee along with his/her problems.

Cont… Refer to secondary/ tertiary care centers Facilitating the accessibility to socio economic rehabilitation services. Review meetings by all stake holders, to discuss progress. Coordination with social welfare department and working jointly

DESIRABLE OUTCOMES People with disabilities receive individual assessments and are involved in the development of rehabilitation plans outlining the services they will receive. People with disabilities and their family members understand the role and purpose of rehabilitation and receive accurate information about the services available within the health sector. People with disabilities are referred to specialized rehabilitation services and are provided with follow-up to ensure that these services are received and meet their needs.

Cont… Basic rehabilitation services are available at the community level. Resource materials to support rehabilitation activities undertaken in the community are available for CBR personnel, people with disabilities and families. CBR personnel receive appropriate training , education and support to enable them to undertake rehabilitation activities.
