Introduction Copper T is an intrauterine device made of plastic and copper It is used for contraception and family planning It is also used for emergency contraception(5 days after intercourse) It is highly effective reversible family planning methods 3

Mechanism Act primarily by preventing fertilization Copper ions decrease sperm motility and function by altering the uterine and tubal fluid environment, thus preventing sperm from reaching the fallopian tubes and fertilizing the egg. The device also stimulates foreign body reaction in the endometrium that releases macrophages and prevents implantation. 4

Types Earlier devices: Cu-7 Cu T 200 Newer devices: Cu T 220 C Cu T 380 A (2002, NFWP,10YRS) Nova T ML-Cu-250(3 YRS) ML-Cu-375(5 YRS) 5

Ideal candidate for IUD use: Having atleast one child No h/o PID, ectopic pregnancy Normal menstrual cycle Access to follow up and treatment In monogamous relationship Willing to check IUD tail Use : during or within 10 days of menstruation- uterine cavity is relaxed, insertion is easy, contractions are minimum so expulsion chance less. 12

Advantages : Effective and reversible method Not complex process in insertion, though supervised Inexpensive if duration is considered Free of side effects(metabolic) One time motivation reqired Highest continuation rate Disadvantages: Contraindication : Absolute Suspected pregnancy/ PID/ Genital Bleeding/ CA of genital tract/ previous ectopic Relative – anemia / menorrhagia / purulent cervical discharge/ h/o PID / Distortion of uterine cavity/ un motivated person 13

Side effects: Bleeding Pain Pelvic infections Uterine perforation Ectopic pregnancy Expulsion – partial or complete Mortality – extremely rare Follow up : examined after her first menstrual cycle, then on third menstrual period, and after that after 6-12 months interval To motivate/ confirm presence of IUD/ diagnose or treat any complication. 14
