Definition: Crowding is the irregularities of teeth due to inadequate space in the arch for normal alignment usually manifested by imbrications, displacement, impaction, rotation, tilts. Crowding and irregularity are the most prevalent component of a malocclusion in dental patient.
CROWDING
Causes of crowding: Dento - alveolar disproportion: -narrowness of the dental base in the labial segment. -short anterior-posterior dental base. Presence of supernumerary teeth Abnormalities in size called macrodontia and shape of teeth. Prolong retention of deciduous teeth Premature loss of deciduous teeth
5.Thumb or lower lip sucking may cause lower labial segment crowding. 6.Forward development position of the buccal teeth. 7.Reduction of arch length due to interproximal caries in the primary teeth.
Classification: Primary - caused by the genetically determined size discrepancy between teeth and jaw . Secondary - shortage of space -Caused by deviating functional condition . Tertiary – irregularities developing in the mandible slightly in the maxillary anterior region during early adulthood and later.
Diagnosis: it may be detected Space analysis Tooth size analysis Radiographic examination.
The severity of crowding can be categorized into- Mild- less than 2mm Moderate- 2 to 4 mm Severe – 5 to 9 mm 4. Extremely – 10mm or more
: The patients age and the likelihood of the crowding increase or reducing with growth. The position, presence and prognosis of remaining permanent teeth. The location of crowding The degree of crowding in millimeters The patient profile. T he following factors need to be considered while planning treatment for crowded teeth
Treatment of crowding : In case of mild crowding -utilization of leeway space -extraction of first molars at early age -distal movement of buccal teeth -slight lateral expansion of arch -using space maintainers to prevent forward movement of first molars.
2. in case of moderate crowding -using removable or fixed appliance after gaining the space 3. in case of severe crowding A. treatment by extraction: - extraction of 1 st molars -retraction of canine by canine retractor - alignment of anterior teeth using labial bow
-retention of teeth using retention appliance such as Hawley’s appliance. B. Treatment by serial extraction . Uses of fixed appliance: offers better control in treatment of crowding by using of multilooped arch wires or nickel titanium wires.
