Carcinoma of female breast is among the commonest of human cancers throughout the world. Infiltrating(invasive)duct carcinoma is the classic breast cancer and is the most common(nearly 80%) histologic pattern of breast cancer.

Risk factors Nulliparity and no or less or late breastfeeding increases the risk of breast cancer. Estrogen excess,as in estrogen replacement therapy in postmenopausal women or the use of oral contraceptives. Environmental and dietary factors such as consumption of large amount of saturated fats,alcohol,cigarette smoking increases the risk. Hereditary- there is increased risk of developing breast cancer in women with affected first degree relatives. Mammry Dyspalsia - women with fibrocystic disease of breast and mammry dysplasia have 2-6 times higher risk of developing breast cancer

Clinical presentation Clinically brest cancer usually presents as a solitary,painless,palpable lump which may sometimes be ulcerated. It is 50% more common in left breast ,in upper outer quadrant. Carcinoma breast is mostly unilateral but may be bilateral. Retraction of the nipple and attachement of the tumour to underlying chest wall may be present. Sometimes patient may present with axillary lymphadenitis.

Gross features Grossly the tumour is irregular,1-5 cm in diameter,hard cartilage like mass in consistency that cuts with a grating sound. Cut surface of the tumour is grey white to yellowish with chalky streaks and often extends irregularly into surrounding fat. Occasionally areas of haemorrhage may be present.

Microscopic appearance Anaplastic tumour cells forming solid nests,cords,tubules are seen invading into stroma with a surrounding desmoplastic(collagen producing)response. The tumour cells are usually pleomorphic,large round or polygonal have deeply stained chromatin in nucleus. Stroma is scanty with lymphocytic infiltration in some cases. Tumour cell invasion in perineural and perivascular space can be seen.
