DISORDERS OF EUSTACHIAN TUBE Tubal Blockage Adenoids and Eustachian Tube Function Cleft Palate and Tubal Function Down Syndrome and Tubal Function Barotrauma

Tubal Blockage Due to tubal blockage O 2 ,CO 2 ,N 2 diffuse in blood. Results in negative pressure in middle ear and retraction of tympanic membrane. Negative pressure further increased and cause locking of tube. Locking occur with collection of transudate and later exudate and even haemorrhage.

Tubal blockage cont. Eustachian tube obstruction can be mechanical, functional or both. Mechanical obstruction :- can result from Intrinsic causes - inflammation Extrinsic causes - tumour in the nasopharynx Functional obstruction :- Caused by collapse of the tube due to increased cartilage compliance. Resists opening of the tube or failure of active tubal-opening. Poor function of tensor veli palatini

Adenoids and Eustachian Tube Function Adenoids cause tubal dysfunction by Mechanical obstruction of the tubal opening. Reservoir for pathogenic organisms. During inflammation mast cells of the adenoid tissue release inflammatory mediators

Cleft Palate and Tubal Function Abnormalities of torus tubarius showing high elastin density. Tensor veli palatini muscle does not insert into the torus tubarius in 40% cases.

Down Syndrome and Tubal Function Function of eustachian tube is defective due to poor tone of tensor veli palatini muscle. Abnormal shape of nasopharynx.

Barotrauma Nonsuppurative condition resulting from failure of eustachian tube to maintain middle ear pressure at ambient atmospheric level. C ause is rapid descent during air flight, underwater diving or compression in pressure chamber
