4 TREND REPORTS: Pest Sightings Note: The basis for the level of infestation (Light, Moderate, Heavy) on the following pages are based on the Infestation Rating Guidelines on Page 3.
5 Date No. of Pests Type of Pest Le vel of Infestation 18-Apr 3 Red Flour Beetle Light 18-Apr 2 Cockroach Light Findings: Red flour beetles and cockroaches were the insects found in the Commissary Building and Perimeter. The total number of pest seen throughout the month of monitoring dropped compared to the last two month. Action Plan/Recommendation: Maintain cleanliness to keep the area low at pest activity. Action Taken: Daily inspection and pesticide treatment is being done by assigned technician . Spraying activity is also being conducted.

8 ILT Monitoring: Analysis Week 1 First week of ILT monitoring, few presence of pests were noted. Flour Beetle, Flour Moth, House Fly, Fruit Fly, Mosquito, and house lizard were the common pests found in the ILT unit. The mentioned pest activity was still at a light/minimal level of infestation. Both ILT units were working properly. Week 2 For the second week of ILT monitoring, pest activity drops compared with the last week’s monitoring. Hence, the mentioned activity was still at a light/minimal level of pest infestation. Both ILT units were working properly. Week 3 During the third week of monitoring in all ILT unit, pest activity noted drops compared with the last two (2) weeks of monitoring. Flour Moth, House Fly, Fruit Fly, Mosquito, and house lizard were the pests recorded. Both ILT units were working properly Week 4 Numerous pest were noted during the 4 th week of ILT monitoring. However, the level of infestation was still at a light/minimal. Both ILT units were working properly. Week 5 Week 5 of ILT monitoring, pest activity noted in different ILT units shows increased compared with the last four (4) weeks of monitoring. However, the level of infestation was still at a light/minimal. Both ILT units were working properly.
9 SUMMARY OF FINDINGS AND RECOMMENDATIONS AREA Commissary Building and Perimeters Thorough cleaning of flour/ingredient spillages to avoid insect habitat. Regular monitoring and inspection must be done to prevent pest infestation. Accumulated dust around the area must be remove immediately. Avoid any food crumbs or any spillages that may attract pests. Regular cleaning must be done in the area to prevent further infestation. Proper waste disposal must be observed.
