Assessment Patient profile Name: Mr. X Age: 26 Chief complain: "my teeth are getting dirty so I want to clean them”. History of complain: since the last 2 months. Medical history: he was smoker but he quit smoking it has been more then a year other then no significant findings. EO/IO: no significant finds. Dental history: he went to dentist for the extraction of his 4 6.
Assessment (cont’d) Patient had no significant findings from the pre assessments Gingival description: generalized uhealthy, localized eurythmic on lower lingual, size is fits snugly, generalized knife like margins and localize roled on lower linguals, papilla is pointed. Consistency is firm, stippled and shiny, with no recession Radiographs: grater then 15 % generalize moderate bone loss and class I and class II carries were also found. Periodontal chart : generalized 2-3 mm and localized 4mm-6mm pocket depths. Dental charting: margin of 3 2 is broken. Plaque index: 75% plaque score on all surfaces. Calculus detection: calculus code 2, in all molars mostly. ASA- I , AAP- II , Cal code- 2
Dental hygiene diagnosis Generalized moderate cronic periodontitis. (1999) Generalized moderate perio, stage II, grade B, currently unstable. (2017)
Planning Teach patient modified bass method brushing technique . Teach patient about flossing. Encourage patient to continue using mouthwash consistently. Full mouth NSPT, polish and fluoride.
Implementation Carried out assessment process (patient profile, P.C, T.C, radiographs, exploring, and probing. on the base of assesments make care plan for the patient care. Did quadrant 1&4 NSPT, on one appointment. Then on next appointment did quadrant 2&3 NSPT. while these appointments i educate patient about brushing technique and flossing. Also recommended soft bristle brush and small amount of floridated tooth past. Polishing and applying varnish.
Evaluation For future maintenance advised patient for follow up at least after 6 months and maintain good oral practices.

Experience with first patient As far as experience it was a huge experience for me, I learn how to take care of a patient while working, how to communicate well so that he’s able to get you, and the most important thing was to develop a relationship of confidence by informing him whatever you’re going to do, and asking the patient if he has any questions or any feed back or and special needs. Then another important thing i learn is to made things ready on time and being pre-planned about what i am going to do and whta i will be doing in future. following the sequence and following time.
Mistakes and learnings Mistakes Learnings Delaying things in the beginning. Time management help a lot Initially having problems in ergonomics which can be harmful for both patient and clinician Maintaining a good and comfortable working position is better for both patient and clinician Rushing in at the ending days Work on time and be prepared for what is coming next. Waiting for instructors sometimes to check Don’t wait just keep going. Missing important things Try to document everything on time
