Sign-in Requirements Partners are required to Sign in and out daily at (at main Gate) Wear ID card at all times, and where it is easily visible

Give your name, location, and a description of the emergency. GCM-SPV is committed to providing a safe and healthy work environment. To help ensure your safety, please familiarize yourself with the site emergency procedures, as well as the locations of all exits and fire pull stations in the building. In Case of Emergency  Evacuation Procedures Evacuate the building immediately when the evacuation alarm is activated. Proceed directly to the evacuation Assembly point. Once at the evacuation Assembly point, listen for instructions and give your name to the Area Monitor conducting the headcount. Do not call the receptionist or building manager to ask if the alarm is real. Do not use elevators. Do not access your vehicle. Do not linger in the lobby or other common areas. Do not re-enter the building until notified to do so by emergency personnel. Non-Life-Threatening Emergencies For after hours, non-life-threatening emergencies (i.e., broken pipes, water leakage, power failures, or strange odors), call the GCM-SPV Help Center: Depot Maintenance Team XXXXXXX

General Safety & Security Guidelines No one will be allowed to enter the site without proper identification Smoking is never permitted inside GCM work areas, but may be permitted in designated outdoor areas Anyone under the influence of alcohol or illegal drugs will not be permitted on site Weapons and firearms are prohibited inside GCM property Cameras and video equipment are not permitted in GCM workspace unless authorized Do not prop open doors; use designated Partner( Cont.Opt,OEM )entrances Park in designated areas

Housekeeping Cleanliness and orderliness are required in work areas.  Partner should: Ensure your work does not impact building occupants Clean up and remove wastes generated on site Clean up rags, packing materials, dust, etc. daily and place in proper containers Avoid placing obstacles in roadways, walkways, aisles or other travel routes Allow sufficient time at the end of the day to clean up work areas Immediately contain any spills, contact Supervisor/Follow Emergency for Spill Release

Personal Protective Equipment (PPE) Appropriate PPE for the hazards associated with the job you will be performing is required: Safety glasses with side shields (ANSI Z87) Substantial or steel toed shoes/boots Hard hats Hearing protection Gloves applicable to hazard ( eg. leather, chemical resistant, cut resistant) Respirators, dust masks and/or face shields Fall protection Electrical insulating equipment ( eg. rubber gloves, blankets, boots, etc.) Welding equipment ( eg. shields, aprons, gloves, etc.) PPE should be inspected periodically to ensure it is in good working condition

Material Handling (Movers) Ensure that tripping hazards are not created in aisles and walkways  Place floor coverings on heavily traveled work areas Avoid placing materials in aisles, walkways, or exit routes Use safe lifting methods to avoid strain or injury

Ladders Metal ladders are only allowed for non-electrical work and above-ceiling work where all hazards have been identified and mitigated. FRP ladders must be inspected before each use.  Ladders should be heavy duty, industrial strength Stepladders must be fully open and cannot be used as straight ladders Defective ladders should be removed from service Do not place ladders in blind spots (doorways, driveways) or in egress ways unless properly barricade, guarded, or signed Straight and extensions ladders must be: Tied-off to keep them secure Extend three (3) feet beyond top landing Set up with the base at least one-fourth of the ladder height measured from bottom to point of bearing

Tools & Equipment Defective tools and equipment should be tagged and taken out of service for repair or disposal Equipment and materials should be stored so that it will not create a falling or tripping hazard or block access to fire extinguishers or emergency equipment Machinery (e.g., portable grinders) should not be operated without effective guards in place Gasoline and propane powered equipment are prohibited inside any GCM building Motor vehicles should not be left running while unattended All moving equipment must be equipped with backup alarms

Compressed Gas Cylinders Store cylinders upright and secured (chained or on cart) Replace valve protection caps when not in use Close valves on empty cylinders Separate oxygen and fuel gas cylinders by a minimum of 20 feet (6 meters) or a 5 foot (1.5 meters) high, one-half hour fire-rate barrier when not in use Use proper equipment when moving cylinders (e.g., cylinder cart)

Fire Prevention Never use gasoline or flammable liquids as cleaning agents Gasoline and similar flammable liquids must be stored in appropriate safety containers and in areas free of burning hazards Keep all heat sources away from flammable liquids, gases, or other combustible materials Open fires are prohibited Hot work operations must have a properly trained and equipped fire watch with fire extinguishers

High Risk Operations/High Hazard Activities

Lockout/Tagout (LOTO) Primary, secondary, stored and single source energy sources require a lockout when performing servicing or maintenance activities Shut down machinery with moving parts or process equipment in service before adjustment or repairs LOTO procedures must be followed and provided by GCM or equipment owner If shut down is not feasible, a risk assessment must be completed and approved prior to start Never remove warning or danger tags or locks Partner must implement procedures that provide protection equal to or better than the GCM LOTO program LOTO programs must be reviewed by authorized GCM personnel Equipment specific Lockout/Tagout (LOTO) procedures must be followed for all LOTO operations Obtain procedures from equipment owners (e.g., GCM or landlord)

Lockout/Tagout (LOTO) Partner( Cont.Opt,OEM ) must Place their own locks and tags (one lock, one key, one person) Verify LOTO by tryout Documentation of LOTO training is required for all Partner( Cont.Opt,OEM )workers performing LOTO

Electrical Electrical equipment should not be installed, repaired, or removed except by trained qualified electricians Electrically operated equipment must be grounded Equipment that is frayed, worn, or otherwise defective should be removed from service Do not use electrical tape to repair or cover damaged insulation on extension cords Extension cords must have a UL approved three prong grounded plug Temporary lighting must be guarded

Working at Heights All workers in an area exposed to a fall greater than 6 feet (1.8 m) (Note: general industry standard is 4 feet, 1.2m) must use appropriate fall protection  Guardrails, Safety nets, Personal fall arrest systems, or Other approved protection method Working at heights is a high-risk activity, each Partner( Cont.Opt,OEM )employee should inspect prior to starting the task.  Working on a roof within 6 feet of the edge or a floor opening requires fall protection Do not stand on motors, pumps, conduits, etc. to gain access to elevated work

Fall Protection - Lifts In scissor or aerial lifts: Safety harnesses must be worn at all times Chains must be closed Harnesses must be secured to an approved tie-off point Tie-off points for aerial lifts should be on the lift to prevent “bouncing out” Tie-off points for scissor lifts should either be outside of the lift or at a manufacturer’s approved tie-off point on the lift Establish a barricaded or roped off danger zone around lifts for falling objects

Scaffolds Scaffolds shall meet GCM or country specific standards Scaffolds must be inspected before use and must be designed for the safe working load Scaffold planking must be: Tested and approved to carry the load Secured by tying or cleats Handrails and toe-boards must be used Rolling towers must be: Locked while in use Free of personnel, material and equipment when moved Moved from the bottom, not the top

Scaffolds Ladders must be used for access, climbing on bracing is prohibited Only trained and qualified personnel should erect, move, dismantle or alter scaffolding Abide by the Scaffold Tag System GREEN – complete per required safety standards YELLOW – conditional use – 100% fall protection required RED – scaffold not complete, Do Not Use Stacked boxes, chairs, or drums cannot be used to make platforms or be added to already built platforms All scaffolds shall be free of tools, trash, etc. before calling in for removal

Cranes, Hoists & Rigging Never raise a load over people or occupied buildings Tag lines must be used to control every load Rigging operations utilizing chains is not permitted without prior EHS approval Multiple-lift rigging is prohibited All materials shall be rigged to prevent unintentional displacement Hooks with self-closing safety latches shall be used Riding on hooks, headache balls or slings is prohibited Defective equipment will be tagged and removed from service Only qualified operators may operate power equipment and seat belts must be worn where appropriate

Cranes & Hoists Safe lifting procedures for cranes and hoists must be developed and documented Equipment inspections are required prior to each use on each shift and as necessary during its use to ensure that it is safe Preventive maintenance is required per manufacturer’s guidance or local requirements The following documentation is required to be provided to GCM-SPV: Training certifications for all crane operators Annual inspection reports Maintenance records

Confined Space Entry into a confined space shall be conducted only if necessary to do assigned work Entry into a confined space is prohibited until atmospheric testing and applicable entry procedures have been documented and permits completed Documentation of confined space training is required for all Partner( Cont.Opt,OEM )entrants and attendants All entrants and attendants must be informed of the entry procedures and mitigation plan prior to entry Retrieval equipment is required for all confined space entries. The number of entrants must equal the number of retrieval devices available and rescue must be available within 4 minutes or the number attendants needs to be increased

Inspections Partner are responsible for performing inspections of their work area and documenting findings daily. Identify hazards & track to closure… Non-compliance to EHS rules may result in disciplinary action GCM- required closure rate is 90% closed in 24 hours, 100% closed in 5 days Disciplinary action First warning – verbal Second warning – written Third warning – termination A Partner worker, for just cause, may be dismissed and permanently refused admission to the project regardless of whether previous violation notices have been issued
