SUBJECT 13: HONORABLE DISMISSAL An approval of transfer of credentials (honorable dismissal) can be either voluntary application or as consequences of a punishment enforced on a student for disciplinary reasons .

Guidelines, Conditions, and Requirements 1. A written petition signed by parent or guardian is required upon application of transfer of credentials. 2. Along with the petition letter, a student must also submit the following documents to the Office of the College Registrar:

a) Duly accomplished “Request for Transfer Credentials” b) Obtain clearance for all accountabilities with the institute c) Official Receipt of Payment

3. Once granted transfer credentials, a student may not be readmitted.

SUBJECT 14: OFF-CAMPUS ACTIVITY The college allows organizations or student groups, like from sections or classes, to conduct off-campus activities that will help to improve the academic, social, physical, mental, and spiritual development of the students.

Guidelines, Conditions, and Requirement 1. Permission to hold an off-campus activity must be requested from the College President at least one (1) week before the event takes place and before notices of the activity are posted or published . 2. An off-campus activity shall oblige the students or participants to fill-out a parental consent form signed by the parent or guardian. A “no parent’s (guardian) signature, no outside trip” policy shall be implemented. The organizer shall provide the College President with a complete list of participants for the off-campus activity.

3. The students must be accompanied by their adviser or a LCCr employee through-out the off-campus activity. 4 . Co-curricular/extra-curricular off-campus activities must be endorsed by the College Dean and approved by the College President. 5 . No major extra-curricular activities of any kind shall be permitted one (1) week before the final examinations and during the final examination week.

SUBJECT 15: THE GRADE CARD At the end of each semester, the college issues to the student an official Grade Card by term. This encompasses the following information about the accomplishment of the student: Student Name Student ID Number Academic Year and Term All courses officially enrolled including the number of units and the official grade Grade Point Average (GPA) on the semester Cumulative Grade Point Average (CGPA ) Date printed
