Fertility and sterility journal A higher number of oocytes retrieved is associated with an increase in fertilized oocytes, blastocysts and cumulative live birth rates

Objective To investigate between the number of oocytes retrieved and the numbers of fertilized oocytes and blastocysts and cumulative and primary transfer live birth rates.

Design Retrospective study.

Study Population Autologous IVF retrieval cycles( IVF, oocyte banking,and embryo banking cycles)and linked FETs from IVF clinics in US between 2014 and 2019. A total of 1,283,131 retrieval cycles and FETs were retrieved from database.

Exclusion Criteria Missing baseline AMH or BMI Cancelled cycles with no egg retrieval When number of 2PNs exceeded the number of oocytes retrieved

Final Number 402,411 retrieval cycles were included from 296,409 patients.

Statistical Analysis The relationship between the number of oocytes retrieved and the number of 2PNs and Blastocysts were assessed using the Pearson correlation and by computing the least -squares best fit . The cumulative and primary transfer LBRs were calculated per the number of oocytes in the associated retrieval . To assess the relationship between the number of oocytes retrieved and LBR metrics , the multivariate mixed effects logistic regression models were trained using the categorical values of oocytes retrieved , pt. age , BMI, AMH and infertility diagnosis as predictor variables and the cumulative and primary transfer LBRs as the response variables.

Correlations between Oocytes and 2PNs/Blastocysts

Primary Transfer LBR The primary transfer LBR per retrieval increased with the number of oocytes retrieved until approximately 16 – 20 oocytes, at which point it began to plateau. Frozen transfers with PGT had highest success rate, especially at lower oocyte yield , and only increased moderately with number of oocytes retrieved. This was followed by Frozen transfer without PGT and then fresh transfers.

Primary Transfer LBR For fresh transfers , the odds ratio of live birth showed: a statistically significant decrease (P <0.01 ) at 2 lowest groups (0-5 & 6-10 eggs retrieved). remained close to 1 between 16 & 30 oocytes retrieved. had a statistically significant decrease (0.01<P <0.05) between 31 – 40 oocytes. For frozen transfers , the odds ratio increased with each successive oocyte group until 26-30 oocytes retrieved for non PGT transfers and until 16-20 oocytes retrieved for PGT transfers . Primary transfer was calculated on a pre – retrieval basis and was defined as a live birth from the first embryo transfer associated with the retrieval .

Cumulative LBR The cumulative live birth rate increased with the number of oocytes retrieved , rapidly to 16 – 20 oocytes , beyond which it continued to increase but with diminishing returns . Using 11-15 oocytes retrieved as a reference group, the odds ratio of live birth increased until 36-40 oocytes. Model coefficients were stastically significant (P<.05) for all oocyte group.

Conclusion A higher number of oocytes retrieved improves the cumulative LBR without imparing the primary transfer LBR. The primary LBR did plateau , but did not substantially decrease with higher oocyte yields , whereas the cumulative LBR increased with the number of oocytes retrieved . The linear correlation between oocytes and 2PNs/ blastocystsmean conversion rate between oocytes to 2PNs/ blastocysts doesnot decline with increased oocyte yield . Therefore , ovarian stimulation strategies should aim to safely maximize the number of oocytes retrieved .
