Warnings
In case severe, unexplained abdominal pain persists or worsens, or occurs
together with symptoms like fever, nausea, vomiting, changes in bowel
movements, abdominal tenderness, decreased blood pressure, fainting, or
blood in stool, medical advice should immediately be sought.
Hyocimax Plus 510 mg tablets should be used with caution in conditions
characterised by tachycardia such as thyrotoxicosis, cardiac insufficiency or
failure and in cardiac surgery where it may further accelerate the heart rate.
Due to the risk of anticholinergic complications, caution should be used in
patients susceptible to intestinal or urinary outlet obstructions.
Because of the possibility that anticholinergics may reduce sweating, Hyocimax
Plus should be administered with caution to patients with pyrexia.

Warnings
Elevation of intraocular pressure may be produced by the
administration of anticholinergic agents such as Hyocimax Plus in
patients with undiagnosed and therefore untreated narrow angle
glaucoma. Therefore, patients should seek urgent ophthalmological
advice in case they should develop a painful, red eye with loss of vision
whilst or after taking Hyocimax Plus.
As the tablet coat contains sucrose (41.2 mg), patients with rare
hereditary problems of fructose intolerance, glucose-galactose
malabsorption or sucrase-isomaltase insufficiency should not take
Hyocimax Plus tablets.

Hyocimax Plus Overdosage
Hyocimax Plus tablets is made up of two medicines: Hyoscine
Butylbromide and Acetaminophen (Paracetamol).
Serious signs of poisoning following acute overdosage of Hyoscine
Butylbromide have not been observed in man.
In the case of overdosage, anticholinergic effects such as urinary
retention, dry mouth, reddening of the skin, tachycardia, inhibition of
gastrointestinal motility and transient visual disturbances may occur,
and Cheynes-Stokes respiration has been reported.

Hyocimax Plus Overdosage
In acute Acetaminophen overdosage, dose-dependent, potentially
fatal hepatic necrosis is the most serious adverse effect. Renal
tubular necrosis, hypoglycemic coma, and thrombocytopenia may
also occur.
Early symptoms following a potentially hepatotoxic overdose may
include: nausea, vomiting, diaphoresis, and general malaise. Clinical
and laboratory evidence of hepatic toxicity may not be apparent until
48 to 72 hours post-ingestion.

Treatment of Hyocimax Plus Overdosage
In the case of oral poisoning, gastric lavage with medicinal charcoal
should be followed by magnesium sulfate (15%). Symptoms of Hyoscine
Butylbromide overdosage respond to parasympathomimetics.
For patients with glaucoma, pilocarpine should be given locally.
Cardiovascular complications should be treated according to usual
therapeutic principles. In case of respiratory paralysis, intubation and
artificial respiration should be considered. Catheterisation may be
required for urinary retention.
In addition, appropriate supportive measures should be administered as
required.

Treatment of Hyocimax Plus Overdosage
If an acetaminophen overdose is suspected, obtain a serum
acetaminophen assay as soon as possible, but no sooner than 4 hours
following oral ingestion. Obtain liver function studies initially and repeat
at 24-hour intervals. Administer the antidote N-acetylcysteine (NAC) as
early as possible. As a guide to treatment of acute ingestion, the
acetaminophen level can be plotted against time since oral ingestion on a
nomogram (Rumack-Matthew).
The lower toxic line on the nomogram is equivalent to 150 mcg/mL at 4
hours and 37.5 mcg/mL at 12 hours. If serum level is above the lower
line, administer the entire course of NAC treatment. Withhold NAC
therapy if the Acetaminophen level is below the lower line.

Hyocimax Plus Tablets During Pregnancy
Hyocimax Plus tablets contain Hyoscine Butylbromide and Acetaminophen
(Paracetamol). Hyoscine Butylbromide and Acetaminophen (Paracetamol) have both
been classified by the US FDA as Pregnancy Category C.
There are limited data from the use of Hyoscine Butylbromide in pregnant women.
Animal studies are insufficient with respect to reproductive toxicity.
In limited reproductive toxicity studies Hyoscine Butylbromide showed no evidence of
teratogenicity in rats at 200 mg/kg in the diet or in rabbits at 200 mg/kg by oral gavage
or 50 mg/kg by subcutaneous injection.
Epidemiological data on oral Acetaminophen use in pregnant women show no increased
risk of major congenital malformations. It is not known whether Acetaminophen can
cause fetal harm when administered to a pregnant woman.
As a precautionary measure Hyocimax Plus tablets are not recommended during
pregnancy.
