TOPIC: IDENTIFICATION OF HEALTH ILLNESS PROBLEMS; SIGN, SYMPTOMS AND BEHAVIORS. PRESENTED TO: RESPECTED ARJUMAND MA’AM. NURSING TUTOR. MMINSR SKIMS PRESENTED BY MEHVISH IQBAL MSC 1 ST YEAR NURSING.

Introduction: Identification of health problems often involves recognizing specific sign, symptoms and behavioral changes. SIGN AND SYMPTOMS:

SYMPTOMS: Remitting symptoms: when symptoms improve or resolve completely they are called remitting symptoms e.g common cold. Chronic symptoms: these are long lasting e.g pain. Relapsing symptoms: these are symptoms that have occurred in past, resolved and then returned e.g major depression disorder.

SIGNS: Prognostic sign: these are the signs that point to future,they predict the outcome for patient such as what is likely to happen to them and how severe the disease will probably be. Anamnestic signs: these signs point to parts of a person’s medical history e.g skin scars may be evidence of severe acne in the past. Diagnostic signs: these signs help healthcare providers recognize and identify current medical problem. E.g urine analysis in UTI. Pathognomonic sign: this means that a healthcare provider can link a sign to condition with full certainty e.g kerning , burdzinski sign.

Depending on the type of illness: Acute illness : days or weeks. Flu: high temperature, chills, sweating, muscle aches, cough etc. UTI: urgency, pain during urination, back pain, pelvic pain etc. Fracture : severe pain, swelling ,bruising, immobility etc. 2 . Chronic illness : months to years. CKD : edema, hematuria , weight loss , lethargy , etc. Diabetes: polydipsia , polyphagia , polyuria etc. Depression: hopelessness, helplessness, worthlessness etc.

So these sign symptoms are body’s way of letting a person know that not everything is running smoothly. Some conditions show no symptoms called asymptomatic conditions e.g HIV . Hippocrates to diagnosis diabetes tasted urine for sweetness, now various diagnostic modalities are available for varied illnesses.

1.History taking: Current concern Past medical history Family history Social history Dietary supplementation Medications (prescribed , OTCs) 2. Physical examination: Head to toe assessment Vital signs 3. Diagnostic testing and imaging: Blood tests like CBC, KFT, LFT, Blood glucose etc. Imaging like X-ray, CT scan, MRI, PET etc.

NURSING DIAGNOSIS: A clinical judgment about individual ,family, or community responses to actual or potential health problems / life processes. NANDA (North American Nursing Diagnosis Association) is the principal organization for defining, distributing and integrating standardized nursing diagnosis worldwide. COMPONENTS: NURSING DIAGNOSIS PROBLEM DEFINING CHARACTERISTICS ETHIOLOGY

BASIC ONE PART: Rape trauma syndrome. Spiritual well being. BASIC TWO PARTS: Difficulty sleeping related to hospitalization. Rise in body temperature related to infection. Risk of fall related to altered consciousness. BASIC THREE PARTS: Ineffective airway clearance related to accumulation of pulmonary secretions as evidenced by crackles on auscultation. Hyperthermia related to underlying infectious process as evidenced by temperature of 100.4˚ F.

BEHAVIORAL HEALTH INDICATORS: Restlessness and agitation Neglect of personal hygiene. Distressed routine. Irrational fear and obsessions. Sleep disturbances. Social withdrawal. Difficulty concentration. Substance abuse. Behavior changes. Suicidal thoughts.

CONCLUSION: Recognizing health illness problems, behaviors and symptoms is critical for early interventions and effective treatment. Awareness of specific symptoms associated with common illness helps in identifying potential conditions early. Regular health checkups, prompt medical consultations, and an open dialogue about mental health and physical health can contribute significantly to improve quality of life. By staying informed and attentive to these signs we can work towards a healthier and more resilient society.
