DEFINITION : Antimicrobial stewardship is an interprofessionl effort Aims at Timely & optimal selection , dose & duration of an antimicrobial agent.

Obtaining best clinical outcome of the treatment or prevention of infection. Minimal toxicity. Minimal impact on resistance.

CDC has defined antimicrobial stewardship ( AMS ) as use of right antbiotic , at right time , with right dose.

Why do we need AMSP? To prevent extensive use of antibiotics is the single most important factor for the bacteria to undergo mutation to become resistant . Misuse & overuse : Overuse for selflimiting infections , nonbacterial infections & treatment of coloniser or contaminant .

Use of antibiotic with overlapping spectra. Goal of AMSP ::: 1. Prevention of emergence of AMR. 2.To reduce colonisation or infection. 3.Improve patient safety 4. Reduce healthcare costs.

IMPLEMENTATION OF AMSP: The key steps of implemetation of AMSP is 1. Administrative support 2.Set up administrative team. 3.Support from microbiology laboratory :

4.Automated bloodculture systems. 5.Automated identification systems. 6.AST.

7..Support from healthcare systems: Automated blood culture systems. Automated identification systems. AST ( Automated ).

From Information Technology: Hospital information system ( HIS). Computer physician order entry (CPOE). Computer decision support system (CDSS).

AMS TEAM: ID Physician Infection control officer. Clinical microbiologist . Antibiotic stewardship nurses. Clinical pharmacists. Officer incharge of pharmacy .
