Documents required for Institute Registration
• Proof of Existence of the Institute: One of the following – Registration with State or Central
Government/PAN No./TAN No.
• Proof of Identity and Date of Birth (of employer): One of the following - PAN
card/Passport/Driving License/Government Id
• Proof of Employership: One of the following - Appointment Letter of employer/head, Board
Resolution, Any Govt./PUC document substantiating proprietorship, Partnership deed
(notorised), Proprietor’s self declaration on institute letter head affixed with institute seal

3 EASY STEPS TO OBTAIN LICENCE/REGISTRATION THROUGH eLORA



Additional requirements for CT and Cath Lab Facilities:
• Prior to apply for Licence, RSO (Radiological Safety Officer) approval is required.
• At least one person who is to be nominated as RSO* of your institute must register
himself/herself as RP (Radiation Professional) in eLORA System. (For ‘Radiation Professional’
registration process, visit eLORA home page ).
* Any Radiologist / X-ray technologists (with three years experience in the field of CT/Cath lab) is eligible
for registering as RP and only after RP registration he/she can be nominated for RSO.

availableonhomepage).Clickon"RegisterInstitute",filltheapplication
formandsubmitalongwithscannedcopyofrequireddocuments
(USERNAMEandPASSWORDwillbesenttoyouremailaddressafter
processingofapplication).
Step1
•EquipmentsDeclaration:AftergettingUSERNAMEandPASSWORDinyour
email,logintothesystemthrougheLORAhomepage.Clickon"Declare
ExistingX-rayEquipment"anddeclareallexistingX-rayequipmentsoneby
one.Obtaininglicencewillbefasterifmodelnameischosenfromthe
availablelist.(Instantlyacknowledgementwillbesentafterdeclaration).
Step2
•RecordofLicence/Registration:IfyouhavevalidAERBLicence/Registration
againstanyofthedeclaredequipment,clickon"RecordLicencefor
operationofX-rayEquipment"andrecordthesamebyuploadingascanned
copyofLicence/Registration(Nofurtheractionisrequiredtillexpiryof
Licence/Registration)
Step3a
•ApplyforNewLicence/Registration: IfyouhavenotobtainedLicence/
RegistrationfromAERBforanyequipment,applyforthesamebyclicking
"LicenceforOperationofExistingEquipment".PriortoapplyforLicence(i)
addsafetytoolsthroughAddInstrumentinInstrumentManagement,(ii)
addradiationworkerandtheirrole(operator/medicalpractitioner)through
AddEmployeeinUserManagement.Afterprocessingandapproval
Licence/RegistrationcanbedownloadedfromeLORAaccount.
