Incident type HiPo#54 Lifting Incident Date/ time 16.09.19@ 20:25hrs Directorate Well Engineering Location Rig 75 Dept UWO PDO Incident First Alert - National Drilling & Services Company Do you ensure you check and confirm all outriggers are out and extended before lifting ? Do you ensure all lifting (routine/non routine) have an approved lift plan? Do you ensure a Lifting Supervisor is supervising the lifting operation? Do you ensure adequate risk assessment is carried out before the of start any lift? Do you ensure the lifting area is barricaded during lifting operations? During preparation of rig up 18 5/8” Casing handling equipment, after offloading the Top Drive System (TDS) bails the crane slew to lift another load, before attaching the load the crane tipped over. There were no injuries in the incident.

Date: 24.04.2019 Incident title: Hipo# 31, OHL lifting I ncident What happened? An electrical crew was engaged in guard pole erection at Hawqa 23 road crossing location. The crane operator raised the pole into the vertical position. While attempting to lift the guard pole, the round endless sling failed to grip the pole and the sling slipped off. The concrete pole toppled onto the crane at the rear side of the driver cabin. Your learning from this incident : Always ensure approved lifting plan is available before commencement of lifting activity. Always ensure that correct rigging method is used for lifting . Always ensure right equipment are in place before commencement of task . Always ensure proper communication and coordination between Appointed Person, Permit Applicant and crew members. As a learning from this incident and ensure continual improvement all contract managers must review their HSE HEMP against the questions asked below. Confirm the following: Do you ensure that appropriate lifting plan is attached along with PTW? Do you ensure the Rigging method used is correct as per procedure? Do your supervisors visit site before applying the permit to see the site conditions? Do your supervisors explain about PTW requirements with appropriate control measures to crew at site? Do you ensure that sub-contractor management is appropriate at site? * If the answer is NO to any of the above questions please ensure you take action to correct this finding. Date: 10.04.19 Incident : HiPo#24 Hoist move MVI What happened? A loaded rig vehicle slightly collided with a 35 ton TANADO crane on Thayfut – Rima black top road. When seeing the rig movement , the crane parked on the road side. The loaded truck while passing the crane hit with the crane operator cabin and the body got damaged. No injury Your learning from this incident: Do not use the road during rig movement. Always stop rig movement during unexpected obstruction on the road. Incident type LTI (03) Date/ time 15.02.19@ 21:15hrs Directorate Well Engineering Location Harweel Dept UWO PDO Incident First Alert - Sea and Land Drilling Do you ensure all hazards are identified before performing the task? Do keep your hands and fingers out of crush points? Do you always consider if you are in the ‘line of fire’? While attempting to pull a snagged line in the crane sheave block a second crane operator’s hand was trapped when the block dropped suddenly injuring his finger. Reconstruction only hook was not on the ground.

What happened? On Friday 10th of January 2019 on the Fahud compression project, a Mechanical crew were engaged in lifting operation using a 3.3 TON / 5.5m side boom loader for 24 inch 24m GRE pipes. While moving the load the lifting belt slipped out of the hook causing one side of the boom to drop which luckily resulted is minor damage and no injury Your learning from this incident.. Always fully comply with the approved lift plan. Use the correct equipment for the correct purpose as described in the approved lift plan. Tool carrier used instead of a crane Incorrect equipment used for the operation Lack of planning & supervision Rigger position in the line of fire. What happened? Safety Alert While unloading 12” x 12 metre pipe from trailer using a 25-tonne crane, the auxiliary hook wire rope snapped and fell to the ground. The hook, weighing 60kg landed 1.5m from the rigger who was at the rear of the trailer. There was no 3 rd party inspection on the auxiliary wire rope for the last 2 years. Safety Alert High potential incident 28.01.2017 Mr. Musleh asks the questions of can it happen to you? Do you make sure the equipment being used has been tested and is fit for use? Do you make sure the equipment and attachments are secure and valid 3 rd party certificates are in date? Do you ensure that the lift plan and the risk assessment are in place? Do you ensure the work area is clear before carrying out lifting operations?

Safety Alert A recent global alert sent from Halliburton is a timely reminder of the importance of routine lifting and offloading practices. In this particular case a driver was severely injured when a pipe was dropped during unloading with a fork lift truck. A number of lessons learned were stated but the most important is keeping the lifting area clear of personnel when the lift commences. We all need to remember: the importance of keeping persons out of the lift area whilst lifting operations are taking place. All riggers and banksmen, tag line personnel and drivers need to be aware of the safe positions before and during the mechanical lift Plan the lift (use the 10 point safety check list) Conduct the Tool Box talk, use pictures or demonstrate to ensure all personnel understand the basic rules IF IN DOUBT…STOP THE WORK AND CHECK IT OUT! Safety Alert 10 point Safety check list for a Safe Lift (PR 1709*) 1. Is everyone aware of and do they fully understand the lifting and hoisting procedures applicable to the lift? 2. Has everyone attended the toolbox talk? 3. Has a pre-use inspection of the Lifting Equipment been carried out and are the Lifting Accessories tagged or marked with: - Safe Working Load - A unique identification number - A valid certification date 4. Are all safety devices working? 5. Does everyone know the Person-in-Charge of the lift? 6. Is everyone competent and aware of his or her tasks? 7. Is there a current Lift Plan and JHA and does everybody understand the job and precautions? 8. Does everyone know the environmental limits (e.g. maximum permissible wind speed) for the lift? Is the lift area controlled and is everyone clear if the load falls or swings?
