Define LGL syndrome The accessory pathway (called James fiber) is intranodal. The accessory pathway bypasses the delay within the AV node. All ventricular conduction occurs through the usual ventricular conduction pathways; unlike WPW, there is no region of ventricular myocardium that is depolarized independently of the rest of the ventricles. Therefore, there is no delta wave, and the QRS complex is not widened. The only electrical manifestation of LGL is a shortening of the PR interval as a result of the accessory pathway bypassing the delay within the AV node.
