Maintenance of Airway and Oxygenation Madam Rozila Binti Ibrahim July 2022
Oral and Nasopharyngeal Suctioning Definition: Process of removing excessive secretions in the mouth and nose of the patients who are unable to clear the respiratory tract on their own (coughing) using Jacques catheter. The patient may have an oral or nasopharyngeal tube inserted. 3/10/2022 Mdm Rozila Ibrahim/MSU/July 2022 2
Purpose of oral and nasopharyngeal suctioning To remove accumulated secretions, blood, vomitus and other foreign material from the trachea. 3/10/2022 Mdm Rozila Ibrahim/MSU/July 2022 3
Requirements Suction machine with appropriate tubing Sterile gloves (various size 6-8) Sterile suction catheter of appropriate size Sterile bowl (for a saline solution to clean the tubing) Sterile saline/water Mask Sterile gauze 3/10/2022 Mdm Rozila Ibrahim/MSU/July 2022 4
Cont.. requirement 10. Biohazard disposable bag 11. Goggles and mask if necessary 12. Gown if necessary 3/10/2022 Mdm Rozila Ibrahim/MSU/July 2022 5
Procedure Assess the patient’s ability to cough and remove secretions. Rationale: To identify the patients requirement for suction 2. Inform and explain the procedure Rational: To reduce anxiety and gain cooperation 3. Perform hand hygiene Rational: To reduce the spread of microorganism 4.Prepare required equipment 5. Put on a disposable apron and protective eyewear if necessary. Rational: To reduce the risk of cross infection and to protect yourself from droplet/sputum contamination. 3/10/2022 Mdm Rozila Ibrahim/MSU/July 2022 8
Cont ..procedure 6. Position the patient into an appropriate position Rational: To enable effective removal of secretion from the respiratory system 7. Connect suction catheter to suction tubing Rational: To enable suctioning of secretion to be carried out. 8. Arrange requirements on the trolley Rational: For ease of the procedure 9. Open the end of the catheter pack 10. Attach the end of the catheter to suction tubing, leaving the catheter within the pack. Rational: To prevent contamination of suction catheter 3/10/2022 Mdm Rozila Ibrahim/MSU/July 2022 9
Cont.. procedure 11. Turn on the suction machine Rational: To enable suctioning of secretion to be carried out 12. Don sterile gloves Rational: To reduces the risk of cross infection to the patient and self. 13. Withdraw catheter from the pack and hold catheter about 4 inches from the tip Rational: For ease of insertion of the catheter into nose/ mouth 14. Ask the patient to breathe in and out deeply 3 times Rational: To help loosen secretions in the respiratory system 15. Ask the patient to cough Rational: To bring out secretion if able to do so. 3/10/2022 Mdm Rozila Ibrahim/MSU/July 2022 10
Cont..procedure 16. Via mouth or nose: Dip the tip of the catheter into sterile saline/water or apply lubricant Rational: To lubricate catheter for easy insertion into oral/nasal cavity Insert the catheter into the mouth/nose gently without applying suction Rational: To prevent injury to the oral and nasal mucosa Apply suction by occluding the suction control port with the thumb Rational: To enable removal of secretion Withdraw catheter using rotating movement Rational: To ensure effective removal of secretions Dip the tip of the catheter into saline/water Rational: To flush secretions within the catheter Repeat suctioning until the chest tract is clear. 3/10/2022 Mdm Rozila Ibrahim/MSU/July 2022 11
Cont..procedure 17. Note color , nature and quantity of secretion Rational: To detect any chest infection 18. Observe the patients respiratory rate, nature Rational: To ensure the patients is breathing normally 19. Rinse suction tubing with sterile water Rational: to ensure sputum is removed from suction tubing 20. Turn off the suction machine 21. Make patient comfortable 22. Clear patients unit 23. Removes gloves, gown and protective eyewear 24. Wash hand 25. Check patients oxygen saturation level half-hour after suctioning 26. Document the time of suctioning and the nature of secretions - nursing documentation 3/10/2022 Mdm Rozila Ibrahim/MSU/July 2022 14
