Information to be made available before an expert accepts responsibilities The responsibility of the experts should be defined before accepting the role of an expert. The timeline and commitment defining the possibility of an expert working on that particular case should be spelt out. The expert should be prompt, and adhering to the timeline is necessary once the assignment has been accepted. Case 1 A 50- year- old obese woman visits her primary care physician complaining of chest pain and shortness of breath. Despite her symptoms aligning with potential signs of a heart condition, the doctor dismisses her concerns, attributing the discomfort to anxiety, stress, and obesity. Over the next few months, her symptoms persist and her husband seeks a second opinion. During the angiography and other investigations, the cardiologist suggests a bypass since he found more than 90% block in three coronary arteries. When asked why the patient was not subjected to angiography since the symptoms were from nearly one and a half years ago. The husband said the primary care physician did not take his wife seriously and blamed the symptoms on her obesity. What is bias? Types of bias How bias can impact health care delivery and health. Case 2 In a popular University, a clinical trial was conducted aimed to test a gene therapy treatment for a rare metabolic disorder. AB was a young man who tragically lost his life during a clinical trial for gene therapy. At the time, he was 18 years old and suffering from OTC deficiency, which affects the liver’s ability to process ammonia. AB volunteered for the clinical trial at the University hoping that the experimental treatment would help him and others with the same condition. Dr CD, who was one of the Principal Investigators held shares in the biotechnology company that owned the rights to the gene therapy technology being tested. He also had consulting agreements and stood to gain financially if the therapy received regulatory approval. 11 March 2024 Training of Experts by EMRB, NMC 15
Contd … The researchers involved did not fully disclose all the potential risks associated with the treatment to AB or his family. AB’s condition was not as severe as initially portrayed. The researchers failed to obtain proper informed consent from AB and made procedural errors during the administration of the gene therapy. During the trial, AB experienced a severe immune response to the vector, and multiple organ failures, leading to his death just a few days after receiving the experimental gene therapy. 11 March 2024 Training of Experts by EMRB, NMC 16
