evolutionwiththeconcurrentapplicationofboth
therapistappliedaccessoryandpatientgenerated
activephysiologicalmovements.

expectedtobeimmediateandsustainedwiththe
applicationofthesetechniqueswithnopain
experiencedduringtheirutilization.

PRINCIPLES OF TREATMENT
Contraindicationstotreatmentexistandshouldberespectedatall
times.
Mustabidebythebasicrulesofapplicationofmanualtherapy
techniques.

BASIC PRINCIPLES (INDICATIONS)
Duringassessmentthetherapistwillidentifyoneormore
comparablesigns.Thesesignsmaybealossofjointmovement,pain
associatedwithmovement,orpainassociatedwithspecificfunctional
activities.

Apassiveaccessoryjointmobilizationisapplied.Thisaccessory
glidemustitselfbepainfree.

BASIC PRINCIPLES (INDICATIONS)….
Thetherapistmustcontinuouslymonitorthepatient'sreactionto
ensurenopainisrecreated.

BASIC PRINCIPLES (INDICATIONS)….
Whilesustainingtheaccessoryglide,thepatientisrequestedto
performthecomparablesign.Thecomparablesignshouldnowbe
significantlyimproved(i.e.,increasedrangeofmotion,anda
significantlydecreasedorbetteryet,absenceoftheoriginalpain).

Failuretoimprovethecomparablesignwouldindicatethatthe
therapisthasnotfoundthecorrectcontactpoint,treatmentplane,
gradeordirectionofmobilization,spinalsegmentorthatthe
techniqueisnotindicated.

Thepreviouslyrestrictedand/orpainfulmotionoractivityis
repeatedbythepatientwhilethetherapistcontinuestomaintainthe
appropriateaccessoryglide.Furthergainsareexpectedwith
repetitionduringatreatmentsessiontypicallyinvolvingthreesetsof
tenrepetitions.

Furthergainsmayberealizedthroughtheapplicationofpassive
overpressureattheendofavailablerange.Itisexpectedthatthis
overpressureisagain,pain-free.

Self-treatmentisoftenpossibleusingMWMprincipleswithadhesive
tapeand/orthepatientprovidingtheglidecomponentoftheMWM
andthepatient'sowneffortstoproducetheactivemovement.

Inthecervicalspine,Mulligandescribesanoscillatorymidtoend
rangemanualtherapytechniqueperformedinseatedweightbearing.
Asthetherapistsglidingforceisalwaysperformedparalleltothe

anaccelerationrear-endautocollisionexperiencesanongoing
painfulandsignificantlossofactiveandpassivecervicalextension.

SNAGSforextensionareareliablemeanstosafelyandefficiently
restoretherangeofcervicalextensionanddecreasethepain
associatedwiththismaneuver.

CLINICAL EXAMPLES…..
Inordertoaccomplishoneorbothofthedesiredtasksofpain
reductionandrecoveryoffunction,thepatientisseatedina
supportivelowbackchairwiththetherapistpositionedbehind.Itmay
beusefultohavethepatientsitfacingalargewallmountedmirroras
itallowsthetherapisttoobservethepatientsfacialexpressionand
thereforereactionduringtheearlystagesofthetechnique.

CLINICAL EXAMPLES…..
Thecurrentrangeofcervicalextension(thecomparablesign)is
checkedforrange,rhythm,deviationandlimitingfactorssuchas;
increasingpainduringrange,painatendoftheavailablerangeor
painlessstiffnessattheendofareducedrangeofmotion.

Thetherapistusesthemedialborderofonethumbreinforcedbythe
padoftheotherthumbtoapplyananterosuperioraccessoryglide
throughthesuperiorspinousprocessoftheinvolvedmotion
segment.

Thedirectionofthismobilizationmustfollowtheplanedescribedby
thesurfacesoftheapophysealjointsundertreatment.
Thecervicalapophysealjointplanesaredirectedantero-superiorly
withtheinclinationincreasingatlowerlevels.

CLINICAL EXAMPLES…..
Whilesustainingthispain-freeaccessoryglide,thepatientis
instructedtoactivelyperformthecomparablesignofcervical
extension.Theexpectationisthattherangedisplayedwill
significantlyincreaseandanypainpreviouslyexperiencedwillbe
virtuallyabolished.

CLINICAL EXAMPLES…..
Asthepatientprogressesthroughtheincreasingphysiological
range,thetherapistmust"track"withthespinousprocessto
maintainhis/herglideparalleltothechangingtreatmentplane.As
such,thetherapists'forearmscircumscribeanarcwhoseapexfaces
antero-superiorly.

CLINICAL EXAMPLES…..
Theendrangephysiologicalmovementissustainedforseveral
seconds.Overpressuremaybegivenattheendofrangebythe
patienttoenhancetheeffectandtheprocedureisrepeatedinsetsof
fivetoten.

Thepatient'spost-treatmentrangeshouldnowbere-evaluatedto
observetheoverallgainwithouttheapplicationofthemobilization
component.

principlesandatoweltoimparttheglidecomponenttomaintain
gainsachievedduringtreatment.

UnilateralSNAGSoverthearticularpillarsmayalsobeattemptedfor
symptomsnotrespondingtocentrallyappliedtechniques.

abilitytotreatapatientdynamicallythrougharangeofmovement.

progressiveandlastingimprovementinrangeanddecreaseinpain.

HIP INTERNAL ROTATION MWM
AlthoughindividualMWMsmaybeappliedforthevariouslossesin
hipjointrangeofmotionobserved,restoringinternalrotationwill
oftenresultinaresolutioninanotherhipmovementdysfunction.

HIP INTERNAL ROTATION MWM….
Thosepatientswithconcurrenthipandlumbarsignsoftenrespond
welltotreatmentinitiallydirectedtothehipthroughthistechnique.
