Psychotherapeutic Approaches 19-10-2025 Manovishada/Depression - Dr Apoorva KS 27

Pharmacotherapy Success depends on correct diagnosis, drug choice, dosage, and treatment duration. With proper treatment, 70-85% of patients can lead a normal life. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 28

COMMOMLY USED ANTI DEPRESSANTS 19-10-2025 Manovishada/Depression - Dr Apoorva KS 29

Response Rate : 60-70 % of patients respond to antidepressants, with full effects typically seen after 4-6 weeks. The choice of drug depends on side effects, medical history, and family history . Side Effects:- Tricyclics : Dry mouth, constipation, blurred vision, weight gain, sedation, and potential cardiac arrhythmias . MAOIs and Lithium: Weight gain and hypotension . SSRIs: Sexual dysfunction, with fluoxetine and sertraline more commonly causing these issues . Others: Bupropion may cause appetite suppression; trazodone can lead to priapism. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 30

Electroconvulsive Therapy (ECT ) Why ? When? Contraindications? Side Effects: Mostly cognitive T emporary confusion and short-term memory loss - To reduce cognitive impairment, ECT is often applied unilaterally to the non-dominant hemisphere, but bilateral treatment may be necessary if the response is inadequate after six sessions . 19-10-2025 Manovishada/Depression - Dr Apoorva KS 31

Indications: Severe depression with psychotic features. Catatonia. Suicidality. Refusal to eat/drink, especially when a rapid response is needed. It is also used in patients with a history of good response to ECT. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 32

Maintenance Treatment for Depression Recurrence: 50-85% ,2-3 years. Recurrence risk: severity of previous episodes, effectiveness of treatment, presence of lingering dysthymia, other psychiatric conditions, or chronic medical issues . Therapeutic Options : 1 . Antidepressants: Continue at full therapeutic dose after recovery . 2 . Lithium: Can be used for long-term maintenance . 3 . Tapering: Gradual tapering is recommended to avoid cholinergic rebound . 4 . Maintenance ECT: For those unresponsive to medications, monthly ECT can be used, with more frequent treatments if necessary . 5 . Interpersonal Psychotherapy (IPT ): May help extend the time between episodes in less severely ill patients not on medication. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 33

Medication Refractive Depression Medication-refractory depression, also known as treatment-resistant depression (TRD), affects about 20-30% of individuals with depressive disorder. Prevalence : Approximately 20-30% of patients with depressive disorder. do not respond adequately to standard antidepressant therapy. Assessment Timeline : A sufficient treatment duration of 6-8 weeks is necessary to determine the effectiveness of a medication before concluding that a patient is non-responsive . Potential Causes of Inadequate Response: Misdiagnosis or incorrect diagnosis. Inadequate dosage or duration of treatment. Coexisting medical or psychiatric conditions. Psychosocial stressors or substance use disorders. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 34

Treatment Options 1. Diagnosis Review 2. Adjunctive Treatment: 3 . Combination Therapy: 4 . Electroconvulsive Therapy (ECT ) 5 . Anticonvulsants 19-10-2025 Manovishada/Depression - Dr Apoorva KS 35

BIOLOGICAL TREATMENTS Folate (vitamin B9) and Cobalamin (vitamin B12) Homocysteine to Methionine Conversion Methylation Reactions Folate Transport in the Brain 2 . S- Adenosyl Methionine ( SAMe ) has shown potential as an effective treatment for depression. Several studies and reviews have demonstrated its antidepressant effects, comparable to traditional antidepressants like tricyclics , with fewer side effects. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 37

3. Omega 3 fatty acids Energy production DNA and neurotransmitter synthesis Myelination (the process of forming a protective layer around nerves ) Phospholipid metabolism 4.Iron The increased stress and Western diet prevalent in the 21st century have been linked to psychiatric illnesses, partly due to the promotion of a proinflammatory state. Supplementation with omega-3 fatty acids (n-3FAs) may help correct this by balancing the ratio of omega-6 (n-6) to omega-3 (n-3) fatty acids, which has become skewed in many modern diets. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 38

NICE guidelines for treatment of depression. Antidepressants are not recommended as first-line treatment in recent-onset, mild depression; active monitoring, individual guided self-help, CBT or exercise are preferred Antidepressants are recommended for the treatment of moderate to severe depression and for dysthymia When an antidepressant is prescribed, a generic SSRI is recommended to patients should be informed about the withdrawal (discontinuation) effects of antidepressants For severe depression, a combination of an antidepressant and CBT is recommended For treatment-resistant depression recommended strategies include augmentation with lithium, an antipsychotic or a second antidepressant Patients with two prior episodes and functional impairment should be treated for at least 2 years The use of ECT is supported in severe and treatment-resistant depression. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 39

Medical and Healthcare Products Regulatory Agency/ Committee on Safety of Medicines Expert Working Group on SSRIs summary Use the lowest possible dose Monitor closely in early stages for restlessness, agitation and suicidality. This is particularly important in young people < 30 years ) Doses should be tapered gradually on stopping Venlafaxine use was originally restricted but this guidance has now been reversed (venlafaxine is probably not cardiotoxic ) but has a less favourable profile in overdose than SSRIs. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 40

1. Meditation. Meditation is the intentional practice of focusing attention. It is of two main forms : 1 . Concentration Meditation 2 . Mindfulness Meditation 19-10-2025 Manovishada/Depression - Dr Apoorva KS 42

2 . Hypnotherapy An intervention using hypnosis , which involves a cognitive process where peripheral awareness and critical analysis are suspended. This can lead to involuntary changes in perception , memory, mood, and physiology. H ypnosis induces a deeply relaxed state, increasing suggestibility and focus. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 43

3. Biofeedback Biofeedback is a therapeutic technique that allows individuals to gain conscious control over normally involuntary physiological functions, such as muscle tension, skin temperature, or brain activity . It uses feedback signals (visual or auditory) to inform the person if a physiological parameter is within a desired range. Over time, through repeated practice, individuals learn to control these responses, which is useful for managing stress and anxiety. Biofeedback often incorporates relaxation and breathing techniques to help reduce stress-related triggers of disorders or diseases. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 44

4. Aromatherapy Aromatherapy originating in cultures such as China, India, Persia, and Egypt . It involves using highly concentrated plant extracts, known as essential oils (EOs ), for therapeutic purposes. These oils are either inhaled or applied topically to promote balance in the body and mind . Aromatherapy can affect mood and behavior , and it has been used to treat conditions such as anxiety , pain, insomnia, dementia-related agitation, and menopausal symptoms. Possible neurochemical effects of aromatherapy include changes in neurotransmitter activity, such as inhibition of glutamate binding and enhancement of GABA and acetylcholine receptor binding. A dverse effects - skin allergies, phototoxic reactions, and increased sedation when combined with certain oils. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 45

5. Spiritual awareness Spiritual awareness is a key component of energy medicine, representing the inner sense of being connected to something much greater and wiser than oneself. It is often experienced in spiritual healing and self-healing practices, where patients may feel a sense of inner peace, spiritual guidance, or reconnection with their spiritual or religious roots. This awareness can arise spontaneously during energy treatments or through deliberate practices like meditation and visualization. Spiritual awareness is particularly beneficial for terminally ill patients and their families, helping them cope with the process of dying. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 46

6. Chiropractic Chiropractic is derived from the Greek words " chieri " (hand) and " praktikos " (to perform), meaning "done by hand .“ It focuses on the belief that misalignments or subluxations of the vertebrae can lead to various health issues, such as back pain, sciatica, muscle spasms, asthma, allergies, and more. Therapists use light touch and manual manipulation to adjust vertebrae, aiming to regulate energy flows and promote the body’s natural ability to heal itself once the subluxation is relieved . Chiropractic care has been found helpful for conditions like fibromyalgia , frozen shoulder, head injury symptoms, and even developmental and learning disorders in children. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 47

7. Qigong Qigong is a traditional Chinese discipline that combines physical and spiritual practices to promote the smooth flow of energy (qi ) throughout the body. Blocked or impure qi is believed to cause illness, and Qigong aims to cleanse or remove these blockages to restore health. The practice involves breathing exercises, meditation, and hands-on energy manipulation along the body’s meridians. Qigong is divided into two main types : Medical Qigong: Used for self-healing or to aid others through techniques like external qi healing, where practitioners focus mental intent to send healing energy . Internal Qigong: A self-healing practice involving adaptable exercises based on an individual's condition and strength.Qigong is beneficial for conditions such as depression , anxiety, insomnia, and migraines . However, excessive or improper practice can lead to psychological or psychosomatic symptoms, and in some cases, Qigong psychotic reaction, a culture-bound syndrome. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 48

8. Music Therapy Music Therapy is the use of music by a qualified professional to improve the psychological, physical, cognitive, or social functioning of individuals with health or educational challenges. Studies suggest that music helps synchronize brain activity across different regions. Music therapy benefits various groups, including children with developmental and learning disabilities , elderly individuals with mental health concerns or Alzheimer's disease, people with substance abuse problems, those with physical disabilities, and individuals experiencing acute or chronic pain, as well as women in labor . 19-10-2025 Manovishada/Depression - Dr Apoorva KS 49

9. Acupuncture Acupuncture is a key component of traditional Chinese medicine ( TCM) involving the insertion of extremely fine needles into the skin at specific points corresponding to bodily functions along energy pathways known as meridians. These meridians are thought to carry energy from the head to the extremities, and acupuncture points can be stimulated not only with needles but also through physical pressure, electrical stimulation, or laser light . The practice aims to release blockages in energy flow, thereby improving the function of related organs. Diagnoses in acupuncture focus on the smoothness of bioenergies ; disruptions in one meridian can affect energy flow in others. Acupuncture has been effectively used to treat depression , anxiety, stress-related issues, and substance withdrawal symptoms. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 50

10. Reiki Reiki is a form of energy medicine derived from the Japanese words rei (universal life energy) and ki (energy), focusing on healing through the laying on of hands and intense mental focus. The core concept of Reiki is to direct healing energy into the body to rebalance blocked energies, release toxins, and activate the body's innate healing capabilities . Practitioners place their hands on or near specific points on the patient's body for up to three minutes without applying pressure. This allows energy to flow into the body, targeting the source of imbalances rather than just the visible symptoms . Reiki has shown beneficial effects, supported by circumstantial evidence from case studies, particularly in managing stress , menstrual issues, migraines, pain syndromes, anxiety, depression, and insomnia. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 51

DISCUSSION Depression is a leading cause of morbidity and mortality worldwide with profound public health concern. In today’s era stress at home, work place, to match pace with others, to reach given targets, constant or recurrent fear of failure or lagging behind , incomplete desires to achieve a goa l are psychological causes of stress and depression. Chronic illness like kustha , pakshaghata , etc also cause depression. Apart from psychological factors, dietary factors like viruddha , asatmya ahara also plays a major role in causing stress. Ayurveda consider sharira and Satva are interrelated with each other and both follow each others pattern in terms of functioning. When sharirika dosha get disturbed eventually mansika dosha (raja and tama) aggravate thus along with physical illness , mental illness also occurs and vice versa. 19-10-2025 Manovishada/Depression - Dr Apoorva KS 57
