Nicardia Retard Tablets(Generic Nifedipine Sustained Release Tablets) are used to prevent certain types of chest pain (angina) and for the treatment of hypertension ( high blood pressure).It can also be used to treat disorder called Raynaud's phenomenon, and premature labor.
This medication may allow you to exercise more and decrease the frequency of angina attacks. Nifedipine belongs to a group of medications known as calcium channel blockers. It works by relaxing blood vessels so blood can flow more easily. This medication should be taken regularly to be effective. Nicardia Retard must not be used to treat attacks of chest pain after they have occurred.

Uses of Nicardia Retard Tablets
Nicardia Retard (Generic Nifedipine Sustained Release Tablets) is
used alone or in combination with other drugs to treat high blood
pressure (hypertension). Lowering high blood pressure helps
prevent strokes, heart attacks, and kidney problems.
This medication is also used for certain types of chest pain (angina).
It may help increase your ability to exercise and decrease the
frequency of chest pain attacks. It should not be used to treat chest

Uses of Nicardia Retard Tablets
Chronic Stable Angina: Nicardia Retard (Nifedipine extended-
release tablets) is indicated in the management of chronic stable
angina (effort-associated angina) without evidence of vasospasm
in patients who remain symptomatic despite adequate doses of
beta blockers and/or nitrates, or who cannot tolerate these
agents.
Hypertension: Nicardia Retard is indicated in the management of
mild to moderate essential hypertension. Nicardia Retard should
normally be used in those patients in whom treatment with
diuretics or beta blocker has been ineffective, or has been
associated with unacceptable adverse effects.

Nicardia Retard Tablets Dosage

The recommended dosage of Nicardia Retard tablets (Generic
Nifedipine Sustained Release tablets) is orally once a day, on an
empty stomach.
Nicardia Retard is a sustained release dosage form and the tablets must
be swallowed whole, not bitten or divided. Normally, titration should
proceed over a 7 to 14 day period starting with 30 mg once daily.
Upward titration must be based on therapeutic efficacy and safety and
will have to be suited to the needs of the patient under treatment.
The usual maintenance dose is 30 mg to 60 mg once daily. Titration to
doses more than 90 mg daily is not recommended.

Storage Instructions

Nicardia Retard Tablets(Generic Nifedipine
Sustained Release Tablets) have to be stored at
controlled room temperature i.e. from 20°C to
25°C (68°F to 77°F) with excursions permitted

Warnings

Excessive Hypotension: Normally in most patients the hypotensive effect of
Nifedipine is modest and well tolerated, occasional patients have had excessive
and poorly tolerated hypotension.
Increased Angina and/or Myocardial Infarction: Rarely, patients, particularly
those who have severe obstructive coronary artery disease, have developed well-
documented increased frequency, duration and/or severity of angina or acute
myocardial infarction upon starting therapy with Nifedipine tablets or at the time
of dosage increase.
Beta-Blocker Withdrawal: When stopping a beta-blocker it is important to taper
its dose, if possible, rather than stopping abruptly before starting treatment with
Nifedipine. Patients recently withdrawn from beta blockers may develop a
withdrawal syndrome with increased angina, which is most probably related to
increased sensitivity to catecholamines.

Warnings

Congestive Heart Failure: Rarely, patients have developed heart failure after
starting Nifedipine.
Concomitant administration with strong P450 inducers, such as Rifampin, are
contraindicated. Nifedipine should not be used in cases of cardiogenic shock.
Since Nifedipine decreases peripheral vascular resistance, careful monitoring of
blood pressure during the initial administration and titration of Nicardia
Retard is recommended.
Mild to moderate peripheral edema occurs in a dose-dependent manner with
Nifedipine.
Clearance of Nifedipine is decreased and systemic exposure increased in
patients with cirrhosis. It is unknown how systemic exposure may be altered in
patients with moderate or severe liver impairment. Careful monitoring and
dose reduction may be necessary in such patients.

Warnings

Do not start therapy with Nicardia Retard (Nifedipine Tablets)
within 3 days of drinking grapefruit juice or eating grapefruit.
Inform your doctor if you have had grapefruit or grapefruit juice in
this time.
Also, do not drink grapefruit juice or eat grapefruit whilst
taking Nicardia Retard Tablets. Grapefruit juice has been
observed to increase the blood levels of the active ingredient,
Nifedipine. This effect can last for at least 3 days.

Overdose
A number of persons have developed toxicity due to acute overdosage
with Nifedipine, either accidentally or intentionally, and via either oral or
parenteral administration. The adverse effects include heart rhythm
disturbances, metabolic acidosis, hypoxia, cardiogenic shock with
pulmonary edema, lethargy, bradycardia, marked hypotension (drop in
blood pressure) and loss of consciousness.
Usually, Nifedipine overdose leading to pronounced hypotension calls
for active cardiovascular support including monitoring of cardiovascular
and respiratory function, elevation of extremities, judicious use of calcium
infusion, pressor agents (medication that tends to raise reduced blood
pressure) as well as fluids.

Nifedipine (Nicardia Retard Tablets) is classified by the US FDA as
Pregnancy Category C.
During animal studies conducted in rodents, rabbits and monkeys,
Nifedipine has been observed to have a variety of embryotoxic,
placentotoxic, teratogenic and fetotoxic effects, including including
stunted fetuses (rats, mice and rabbits), digital anomalies (rats and
rabbits), rib deformities (mice), cleft palate (mice), small placentas
and underdeveloped chorionic villi (monkeys), embryonic and fetal
deaths (rats, mice and rabbits), prolonged pregnancy (rats; not
