Characteristics:
Chronic Disease, definition,characteristics , Focus Definition: A specific type of chronic condition that is characterized by a derangement in biological function or structure, usually diagnosed with specific criteria. Characteristics Often has well-defined pathophysiology. Requires ongoing medical management and intervention. Examples: diabetes mellitus, chronic obstructive pulmonary disease (COPD), heart disease. Focus: Disease management, prevention of complications, adherence to treatment protocols.

CASE SCENARIO Transferring a patient from the emergency department to the surgical unit. ISBAR Breakdown: I: "Hello, this is Nurse Jones from ED." A: "Currently, he is anxious but responds well to reassurance." R: "I recommend preparing a pre-op briefing for the patient."
CASE SCENARIO A patient, Ms. Garcia, diagnosed with metastatic cancer, is feeling hopeless. Demonstration: Active Listening: "I can hear that you feel overwhelmed. Can you tell me more?" Empathy: "It's understandable to feel this way given what you are going through." Open-Ended Questions: "What are your biggest concerns about your treatment?" Validation: "Your feelings are valid, and it’s important we address them together."
