INTRODUCTION In developing countries, like india partograph is being widely used during labour process. It is a very useful instruments that provides optimum results if employed properly in labour management by obstetrics caregiver. Partograph is a graphical record which is used for monitoring the process of first stage of labour.
DEFINITION Partograph is prepared on single page by the obstetrics caregiver which is helpful for her in monitoring the progress of labour. It also provides information about health status of the woman and her fetus .
PURPOSES To detect abnormal progress of labour at early stage. To make the assessment of pregnant woman and her fetus more qualitatively and regularly during labour process. To make appropriate decision during abnormal progress of labour that is refer, augmentation or termination.
COMPONENTS IDENTIFICATION DATA OF PREGNANT WOMAN Name of woman Name of husband Age Parity Registration number Date & Time of admission Date & Time of Rupter of Membrane
CONT… 2. Fetal heart rate 3. Colour of amniotic fluid – It is mentioned as follow: - If fluid is clear then ‘C’ - If it is meconium stained then ‘M’ - If fluid is absent even after rupture of membranes the ‘I’ 4. Cervical dilatation 5. Number of uterine contraction per 10 minute and intensity like, mild, moderate, severe. 6. Status of membrane like, Ruptured or Intact
CONT… 7. Drugs and IV fluid given to the pregnant woman. 8. Maternal BP and pulse rate 9. Maternal temperature.
ALERT LINE & ACTION LINE In the partograph, in the section of cervical dilatation, there are two diagonal lines – ACTION LINE ALERT LINE
CONT… The alert line starts at 4 cm of cervical dilatation, travels diagonally upwards and ends at 10cm dilatation. Parallel to alert line on right side, action line is drawn 4 hour to the right of the alert line. These two lines are designed to warn the obstetrics caregiver to take appropriate step if any abnormal condition arises.
CONT… In case of normal progression of labour, the line indicating the cervical dilatation should be either on left side of the alert line or on the line. If this line is found between the alert and action line, then labour progress should be assessed closely. It indicates an abnormal condition. It the line is found right of the action line than it indicates an emergency condition is to be made either for induction of labour or caesarean section.
ADVANTAGES It is helpful in early recognition of feto -maternal problems It is helpful in reducing incidences of prolonged labour and caesarean section. It is helpful in detecting abnormal progress of labour at early stage. It guided the obstetric caregiver in making appropriate decision during abnormal that is refer, augmentation or termination.
