PEDIATRIC GLASGOW COMA SCALE (P-GCS)

LATAR BELAKANG Salah satu parameter GCS adalah penilaian respons verbal terbaik, yang seringkali tidak dapat dilakukan pada bayi/anak kecil. Modifikasi GCS (Pediatric-GCS) diperlukan untuk anak yang belum memiliki kemampuan verbal. TUJUAN Setelah menyelesaikan pelatihan ini, peserta dapat memahami dan melakukan penilaian Pediatric GCS (P-GCS).

PROSEDUR TEKNIK Mencatat atau memeriksa ulang identitas pasien secara jelas 1 Eye opening response (max score 4) 2 4 = spontaneously eyes opening 3 = eye opening in response to speech or shout 2 = eye opening in response to painful stimuli 1 = no response

Assess grades of best verbal response (max score 5) in verbal or non-verbal child 2 5 = oriented and converses – patient knows who and where they are, & why, & the year, season & month 4 = disoriented and converses - patient responds in conversational manner, with some disorientation 3 = inappropriate words or speech - random or exclamatory speech, with no conversational exchange 2 = incomprehensible sounds - no words uttered, only moaning 1 = no response Verbal child

5 = smiles, oriented to sound, follows objects, interacts 4 = consolable when crying and interacts inappropriately 3 = inconsistently consolable and moans; makes vocal sounds 2 = inconsolable, irritable and restless; cries 1 = no response Non-verbal child

Assess grades of best motor response (max score 6) 3 6 = normal spontaneous movement 5 = withdrawal to touch 4 = withdrawal to pain - pulls limb away from painful stimulus 3 = flexor response to pain - abnormal flexion (decorticate rigidity) 2 = extensor posturing to pain – extension (decerebrate rigidity) 1 = no response to pain

INTERPRETATION - Min. score is 3  the worst prognosis - Max. score is 15  the best prognosis - Scores of 7 or above  a good chance for recovery - Scores of 3-5 are potentially fatal, especially if accompanied by fixed pupils or absent oculovestibular responses or elevated intracranial pressure - Normal children < 5 years may have lower scores than adults because of reduced best verbal and motor responses Catat hasil penilaian di rekam medik , contoh : Tgl . 13 Agustus 2007 Nama pasien : An.
