Rh INCOMPATABILITY IN PREGNANCY MRS U SREEVIDYA PROFESSOR

DEFINITION "Rh incompatibility is a condition when a woman and her unborn baby carry different Rhesus (Rh) protein factors, this condition is called Rh incompatibility. This occurs when a woman is Rh-negative and her baby is Rh positive, the Rh factor is a specific protein found on the surface of red blood cells". OR "Rh diseases (also known rhesus isoimmunization) is a type of haemolytic disease of the newborn. It can be mild, moderate or severe and specifically occurs only in some second or subsequent pregnancies of Rh negative woman when the foetus's father is Rh positive leading to a Rh positive pregnancy. During birth, the mother may be exposed to the infants blood and this leads to the development of subsequent Rh pregnancies".

Rh-blood Grouping System 1. Rh blood group was first demonstrated in rhesus monkey. 2. Blood groups are classified as Rh positive and Rh negative. 3. The Rh factor, (Rh positive and Rh - Negative) usually refers specifically to the presence or absence of antigen-D. 4. There are two alleles, or genetic variants, of this antigen D and dd. 5. A person who is Rh negative has two recessive tracts dd. 6. Anyone who has at least one D-DD or D d-is Rh positive.

EFFECTS OF Rh ISOIMMUNIZATION ON PREGNANCY Usually the effect is seen more on foetus and neonate. However, the following effects are observed in the mother: Preeclampsia Polyhydramnios Large baby in uterus Hyperfibrinogenaemia (when dead foetus is in uterus) Postpartum hemorrhage Big placenta Blood coagulopathy

EFFECTS OF Rh ISOIMMUNIZATION ON NOENATE Rh isoimmunization causes the following effects in a neonate: Haemolytic anaemia Haemolytic jaundice Congestive heart failure Perinatal mortality
