12. What is a thyroid nodule needle aspiration? How is it being done? The starting point: there is a nodule or lump or mass on the thyroid gland which may be palpable or not. Thyroid Nodule Needle Aspiration

15. What is a thyroid nodule needle aspiration? How is it being done? As mentioned earlier: The starting point: there is a nodule or lump or mass on the thyroid gland which may be palpable OR NOT. Thyroid Nodule Needle Aspiration For non-palpable thyroid nodule or nodule which are difficult to palpate, the ultrasound machine can be used to assist in the needle aspiration.

16. What is a thyroid nodule needle aspiration? How is it being done? Palpable thyroid nodule needle aspiration can be done in the clinic, with either the patient sitting or lying down, and the procedure proper of aspiration can be completed within 1-2 minutes. Thyroid Nodule Needle Aspiration

Specimen obtained from a thyroid nodule, especially, if it is solid, is submitted for a biopsy study (microscopic study by a pathologist). Thyroid Nodule Needle Aspiration Note: if the thyroid nodule turns out to be a colloid cyst after aspiration, there is NO need to submit the fluid for biopsy.

18. What are the potential side effects or complications? INVARIABLY, PAIN from the needle puncture as it is inserted into the thyroid nodule (pain is minimal with same level as that in blood extraction with no need for anesthesia and pain killer drugs after) Thyroid Nodule Needle Aspiration

POSSIBLE side effects or complications but rarely happen: Bruising – hematoma (uncommon – usually spontaneously resolved) Tracheal puncture (rare – just observe) Infection (rare – no need for antibiotics) Needle tract implantation of thyroid cancer (0.02% to 0.19%) Thyroid Nodule Needle Aspiration

Starting point for considering needle aspiration: THYROID NODULE!!! in which malignancy is a possibility and one has to know whether it is benign or malignant as there is marked difference in treatment. NOT DIFFUSE THYROID ENLARGEMENT (which usually is suggestive of hyperthyroidism and hypothyroidism) [rarely is thyroid aspiration indicated] Thyroid Nodule Needle Aspiration

21. What are its indications? FOREMOST INDICATION: A THYROID NODULE in which the considerations are either a malignancy or a non-malignancy is present. Treatment is usually markedly different for malignant and non-malignant nodule. There is a need to be definite in the diagnosis prior to institution of treatment. Thyroid Nodule Needle Aspiration

22. What are its indications? OTHER INDICATION: A big or huge COLLOID CYST in which needle aspiration can provide comfort to the patient (as a result of decrease in size) and may facilitate medical suppressive therapy. Thyroid Nodule Needle Aspiration A thyroid nodule needle aspiration can simultaneously be therapeutic if the thyroid nodule turns out to be cystic (colloid cyst) and diagnostic ( to know exactly what the nodule is).

23. How does one interpret the outcomes of a thyroid nodule needle aspiration? If colloid fluid (brownish or yellowish) is obtained (it is a cystic nodule) and there is NO residual mass (complete disappearance of nodule), diagnosis: COLLOID CYST No need to submit specimen for biopsy. 24. How does one interpret the outcomes of a thyroid nodule needle aspiration? If colloid fluid (brownish or yellowish) is obtained (cystic) and there is a residual mass, diagnosis is COLLOID ADENOMA (benign). Specimen obtained from the solid element of the nodule can be submitted for biopsy. Thyroid Nodule Needle Aspiration

25. How does one interpret the outcomes of a thyroid nodule needle aspiration? If NO colloid fluid (brownish or yellowish) is obtained, nodule is solid, cells are obtained, smeared on a glass slide, to be submitted for biopsy. May be benign or cancer. Note: at times, with experience, based on appearance of cells on the smear, I can tell the diagnosis (whether benign or malignant). However, I still wait for the pathologist’s report for the final diagnosis. Thyroid Nodule Needle Aspiration
