School Form 2 Daily Attendance Report for Learner
This replaced Form 1 - School Register (Daily Attendance) and Form 2 – Teacher’s Monthly Report of Enrolment and Attendance
Step 1: Upon checking of attendance in the morning, if the pupil is not around, Mark the upper half of the box with a “\” . (This would presume that the pupil is absent in the morning)
Step 2: If the pupil reports to school After checking of attendance, shade The upper half of the box to indicate TARDINESS
Step 3: Same procedure with Steps 1-2 for the pm class.

Use a separate SF-2 sheet for cutting class.
For cutting classes made in the morning, Indicate the time the pupil cut class In the upper half of the box. 9:00-10:00
For cutting classes made in the afternoon, Indicate the time the pupil cut class In the lower half of the box. 2:00-3:00
Entries in both upper and lower half Parts of the box would indicate that the pupil has made cutting classes, both In the morning and in the afternoon classes. 9:00-10:00 2:00-3:00
Why do we need a separate sheet for cutting class ? There is no other way by which this data can be accurately marked/recorded in the first SF-2. For reference purposes it is a must that cutting classes of students should be noted in SF-2. Proper accounting of students attendance in all classes within the day must be recorded.
Pls take note that we are counting the numbe r of pupils/students per day, hence, a pupil who was only present for half of the day will be treated as absent.
But for absences and tardiness, 0.5 or ½ is still accounted for.

Learners who reported to school beyond the cut-off of BoSY . This reflects only the number of late enrollees during the month being reported and not the cumulative total.
The actual # of learners who are officially enrolled and registered ( incl Transferred In) as of the last day of classes for the month being reported. Learners who are dropped/ transferred out during the month prior the last day of classes for the month must not be added.

Learners who left school to enter another school as evidenced by a request for permanent record (Form 137) .
Additional Guidelines Every End of the month, the class adviser will submit this form to the office of the principal for recording of summary table into the School Form 4. Once signed by the principal, this form should be returned to the adviser . To be accomplished in duplicate. One copy for class adviser, one copy for School Head
Additional Guidelines Attendance performance of learner is expected to reflect in Form 137 and Form 138 every grading period

This form shall be accomplished every end of the month using the summary box of SF-2 submitted by the class advisers to update figures for the month.

Total number of learners who left school in the previous months E ntries are copied from the previous month’s (A+B column) SF-4

For multigrade/combination classes, CLEARLY indicate it by placing brackets
No need for sub-totals per grade/year level. No sub-total for Primary or Intermediate Levels. This summary must be reflected on the last page of SF-4.

Furnish copy to Division Planning and Research in triplicate copies on or before : July 7 (SF-4 for the month of June) November 7 (SF-4 for the month of October) April 7 (SF-4 for the month of March)
