In some respects diagnostic assessment is similar to
formative assessment in that it examines the knowledge
and skills that a pupil has already learnt. It is usually
carried out at the start of a new topic to identify what
pupils already know and can do, enabling the teaching
content of the new topic to be planned.

CONCLUSION
There isn't a massive difference between word meanings. An
assessment tends to be a look at certain areas of how
something is functioning (a student in a class, a work safety
assessment etc.) and the word 'evaluation' has more to do
with the conclusions drawn from that assessment -"he
conducted an assessment and wrote an evaluation of his
findings". However, you could say either "he assessed the
situation/he evaluated the situation" and the difference
wouldn't be substantial.
