INTRODUCTION Shock is a life threatening medical condition where by the body suffers from insufficient blood flow the throughout the body (circulatory collapse) it is characterized by inadequate tissue perfusion in which there is insufficient delivery of oxygen and nutrients to the body’s tissue and inadequate removal of waste product from the tissues if untreated ,results in cell death this can damage multiple organs. Failure of the normal regulatory mechanisms in the body can lead to rapid and profound shock .shock often accompanies severe injury or illness

DEFINITION According to javed ansari :- shock is a condition in which the cardiovascular system fails to perfuse tissue adequately According to Nima schwartz’s :- Shock may be defined as inadequate deliver of oxygen and nutrients to maintain normal tissue and cellular function

INCIDENCE Most couses reported in western countries still involve menstruating women under age 30. TSS still occurs in about 17 out of every 100,000 menstruating girls and women eases are related to tampons between 5% and 10% of patients with TSS die

Etiology & Risk Factors

HYPOVOLEMIC SHOCK It is the most common type of shock is characterized by decreased inter vessel blood volume contained in the intravascular and extra-vascular compartment

CARDIOGENIC SHOCK Shock resulting from inadequate pumping action of the heart because of cardiac muscles disfunction or mechanical obstruction to the blood flow. Cardiogenic shock is a state in which the heart is been damage so much that it is unable to supply enough amount of blood to the organs.

Distributive shock Shock resulting from inadequate vascular tone and the blood volume remains the same however the size of vessels having vasodilation They are mainly 3 types Septic shock Neurogenic shock Anaphylatic shock

SEPTIC SHOCK It is the most common shock that is caused by pathogen in blood endotoxins (it is the type of infection of blood ) . It released vasodilator they also activate complement pathway which stimulate master cell (histamine) another vasodilator . In septic shock the blood flow is high due to this heat production increased by the body so distributive shock is also known as warm shock.

This is caused by allergic reaction of the body against some microorganism It may also lead to low blood pressure ANAPHYLACTIC SHOCK

NEUROGENIC SHOCK In neurogenic shock the nervous system can damage due to some incident or accident lead to low blood pressure .

OBSTRUCTIVE SHOCK:- Obstructive shock is used as another category of shock to include those conditions that leads to a sudden obstruction of blood flow . Obstructive shock occurs when there is a blockage of blood flow in the cardiovascular circuit outside the heart.

Stages Of Shock

PATHOPHYSIOLOGY:- Due to risk factor or precipitation causes of shock Decreased circulatory blood volume which leads to decreased cardiac output Cells lack an adequate blood supply and are deprived oxygen and nutrition

Because of these changes normal cell function cease causes cell membrane dysfunction The cell swells and its membrane become more permeable and failure of sodium pump Toxic substance enter circulation and damage capillary endothelial Death of cell occur (shock)

Bluish nail . Anxiety, Chest pain, Confusion, Urine output,, Unconsciousness ( coma)

Medical Management Respiratory support Circulatory support IABP

PHARMACOLOGICAL MANAGEMENT Vasoconstriction Vasodilators . .

Nursing Management To improve oxygenation. Temperature monitoring. Cardiac monitoring. I/V fluid administration. Psychological support. Blood transfusion.
