Surgical Instruments

OBJECTIVES At the end of this session the student should be able to: Name surgical instruments Discuss The different classifications of the instruments. Mention t he different areas used for these instruments.

Classification of instruments Cutting and Dissecting Grasping and Holding Clamping and Occluding Exposing and Retracting Probing and Dilating Viewing Suctioning and Aspirating Suturing and Stapling Measuring Accessory Micro-instrumentation/

IMPORTANT TERMS Atraumatic : without trauma. Traumatic : Causing Injury by penetration or crushing. Dilation : Enlarging an opening in a progressive manner. Dissection : Process of separating tissues through anatomic planes by using sharp or blunt instrumentation. Grasping : Holding in a traumatic or atraumatic way. Retraction : stabilizing a tissue layer in a safe position for exposure of a part.

IMPORTANT TERMS Sharp : Instrument with a cutting edge or pointed tip ; that is used to cut or dissect tissue. blunt tips : the tip is not sharp Finger Ring : Place for the user to pace his finers Ratchet :( catch lock) Allows instruments to be locked in place. Shank : Connects the boxlock to fingers ring Boxlock : Controls the jaws on the instruments. Jaws : using p art of the instrument it may serrated or smooth, it can be shaped the way of using instruments Tips: The end part of instruments and how the tip is designed and used. Tip

Parts of surgical instruments

Cutting Cutting instruments These instruments are used for incising or dissecting tissues. They have sharp edges Scalpel, Bard parker (BP) handle & blade. Knife Scissors

1-Bard parker (BP) handle & blade . BP handles is used for incision on skin ,fascia ,muscles ,peritoneum etc . The surgical blade is 20 & greaer than fixes No: 4 The surgical blade no 3.5.7&9 fixes less than 20 surgical blades,

Blades in deferent Size

Disposable surgical knifes The most common blades are blade Hamby's Blair’s knife

Skin graft knifes Goulian Skin graft knife, blade and Guard knife set Watson skin graft knife And blade

Amputations knifes

scissors Scissors may be short or long straight Curved Angled Scissors are used for cutting the tissues in various structures (curved). sutures (straight) ligatures (straight ) gauze (Angled, straight) dressing pads (Angled, straight) bandages etc. (Angled, straight)

Mayo scissors Mayo curved are used for cutting tissues & internal organs Mayo Straight is used cutting sutures

Metzenbaum scissor Metzenbaum scissor i s designed for cutting delicate tissue. The Size are13,14,18,20 a nd 23cm

The McIndoes Scissor The McIndoes Scissor are used in Tissue Dissection

Lister Bandage Scissors Used to remove bandages and dressings Probe tip is blunt; inserted under bandages with relative safety

Nelson scissor Nelson blunt-pointed, curved or straight-bladed, long-handled scissors designed for thoracic surgery.

Iris Suture Scissors Used to remove sutures Blade has beak or hook to slide under sutures

Dissecting Forceps Used to hold tissue in place when applying sutures Used to gently move tissues out of the way during exploratory surgery Used to insert packing into or remove objects from deep cavities

1x2 Toothed D issecting Tissue Forceps Used for grasping and holding tissue, muscle or skin surrounding a wound Single toothed on one side; fits between two teeth on the other side Available as 1x2 or 2x3 or 3x4

Dissecting forceps 1-RUSSIAN Tissue Forceps(15-25cm 2-Gillis dissecting forceps

Dissecting forceps 5-BONNEY Tissue Forceps 18cm 3-WAUGH Dissecting Forceps 20cm 4-LANE Tissue Forceps

Dissecting Forceps 6 -ADSON Dissecting Forceps 12cm 7 -ADSON-BROWN Tissue Forceps 9x9 teeth 12cm   8 -McINDOE Dressing Forceps 15cm

Sponge Forceps Used to hold gauze squares or sponge to “mop up” the surgical site Jaws are serrated 1-Foerster Sponge Holding Forceps straight

Sponge holding forceps 2-RAMPLEY Sponge Holding Forceps 24-25cm 3-BONNEY Uterine Packing Forceps 24cm

Hemostatic artery forceps Used to hold delicate tissue Used to compress bleeding vessels Jaws are fully serrated 1-Mosquito

Hemostatic forceps 2-Crile hemostatic forceps ( curved and straight) Used to hold tissue Used to compress bleeding vessels Jaws are fully serrated,

Hemostatic artery forceps 3-Dunhill Artery forceps

Hemostatic artery forceps Used to clamp small vessels or hold tissue Jaws are partially serrated Curved or straight 3-Kelly Hemostat

SPENCER WELLS Artery Forceps 1 -SPENCER WELLS Artery Forceps 2-ROCHESTE R - PEAN Arter y Forceps SPENCER WELLS Artery Forceps are used grasping big artery forceps

SPENCER WELLS Artery Forceps 3-ROBERTS Artery Forceps curved 22.5cm MOYNIHAN Clam p Forceps 19 and 23cm

Right angled artery forceps 1-MIXTER Dissecting Forceps 22.5cm 2-LAHEY Dissecting Forceps with longitudinal serrations

Tissue forceps LITTLEWOOD Tissue Grasping Forceps 18cm   BABCOCK Tissue Grasping Forceps ALLIS Tissue Grasping Forceps LANE Tissue Grasping Forceps 14cm,19cm

Greenarmytage This forceps is used as a  hemostat in caesarean operation I t is applied to the two angles and lower and upper edge of the incision. GREEN ARMYTAGE Hemostasis Forceps 21cm

Duval tissue forceps A light weight forcep with a large grasping surface to distribute force and reduce trauma to tissues. Often useful for grasping the stomach or lung.

Allis tissue forceps It has broad teeth & is used for holding large mass tissue

Lane tissue forceps It is like artery forceps but have single pointed teeth on either side of the inner surface of serrated tip. This allow to hold the small mass tissue or to hold the edge of incision

Babcock This forceps is used for grasping of smooth delicate tissue. It has fenestrated jaws to hold bowels or fallopian tube

Doyen Intestinal Forceps   Doyen intestinal forceps are non-crushing intestinal occluding forceps with longitudinal serrations. Used to temporarily occlude lumen of bowel.

Payr Pylorus Clamps Payr pylorus clamp is a crushing intestinal instrument. Used to occlude the end of bowel to be resected.

Needle Holder needle Used to hold and pass a suturing through tissue Has groove to hold needle within jaws

Cont …. Mayo Hagar needle holder, used for suturing Mayo’s needle holder

Cont.. CRILE-WOOD-TC Needle holders OLSEN-HEGAR Needle holders with Scissors Used for suturing and cutting together

Retractors Retractors: These instruments re used to retract the wound edges & expose the cavity. These are again of various types: Manual retractor (hand hold) Self retaining retractors
