INTRODUCTION Epilepsy is the fourth most common chronic neurological disorders globally. Epilepsy affects approximately 50 million people worldwide with nearly 80% of them in developing countries. In India it is estimated that there are more then 10 million persons with epilepsy. Epilepsy can be successfully controlled with medication adherence, and most people with epilepsy (almost 70% of patients) can become seizure-free by takingdrug (AED).
Non-adherence to medication is a very common phenomenon in patients with chronic diseases including epilepsy. This leads to uncontrolled symptoms and substantial deleterious effects on the quality of life (QoL) and those patients can become a heavy burden on society and healthcare systems. However, there is sparse data available regarding the same in Indian patients, especially those residing in Jammu and Kashmir. In this context, the present study is planned to assess the medication adherence, quality of life in patients of epilepsy and to evaluate the association between the two.
Research question: What is the effect of medication adherence on Quality of Life in patients of epilepsy on treatment for more than 6 months presenting to General Medicine or Psychiatry OPD Government Medical College, Anantnag.
Aims and objectives: The aim of the present study is to assess the medication adherence and quality of life in patients of epilepsy presenting to OPD, department of General Medicine and Psychiatry GMC, Anantnag. The objectives of the present study are: To study the level of medication adherence in patients of epilepsy on treatment for more than 6 months. To study the quality of life in patients of epilepsy on treatment for more than 6 months. To evaluate the association between medication adherence and quality of life in patients of epilepsy.
Material and Methods Study type and design: A cross-sectional observational study with consecutive sampling. Study area : The study will be conducted in the outpatient department supervised by the Department of Psychiatry and Department of General Medicine respectively of the Government Medical College, Anantnag, Jammu and Kashmir
Study period: The study will be conducted over a period of 18 months from the approval of the proposal. Study population: The study population will be made of all adult patients of age 18-60 years diagnosed with epilepsy who have been on treatment for the same for a period of 6 months or longer and presenting to GMC, Anantnag for check-up and/or management.
Inclusion criteria: All the patients diagnosed with epilepsy, confirmed by a neurologist or psychiatrist. Patients receiving treatment for a period of > 6 months presenting in the OPD, Department of Psychiatry and Department of General Medicine Government Medical College, Anantnag Age ≥18 years to 60 years (Adult population) Both sexes. Consenting Individuals.
Exclusion criteria: Patients <18 years and >60 years Patients with Severe Mental and Medical illness. Patients who had a seizure in the previous 24 hours.
Tools to be used: Semi-structured Performa. Socio-demographic questionnaire. BJ Prasad scale. The World Health Organization Quality of Life (WHOQOL-BREF) scale.(English and Urdu translation). Medication adherence rating scale (MARS).
Procedure: PATIENTS OF EPILEPSY ON TREATMENT PRESENTING TO THE OPD FOR FOLLOW UP INCLUSION AND EXCLUSION CRITERIA CONSENT OF THE PATIENT INTERVIEW THE PARTICIPANTS TO GATHER DATA PERTAINING TO THEIR SOCIODEMOGRAPHIC CHARACTERISTICS AND THE SEMISTRUCTURED PERFORMA. CATEGORIZE THE PATIENTS INTO ADHERENT AND SUB OPTIMALLY ADHERENT USING MEDICATION ADHERENCE RATING SCALE (MARS) ASSESSMENT OF QUALITY OF LIFE OF THE TWO GROUPS OF PATIENTS USING THE WHO-QOL-BREF SCALE DATA ANALYSIS
Ethical considerations: The study will commence after obtaining proper ethical permission and clearance from the Institutional Ethics Committee of GMC, Anantnag. Following that, for every participant, written informed consent will be obtained before their recruitment in the study. All participants’ information will be kept confidential and anonymous, and stored in a password protected document. Statistical analysis: The statistical analyses of the data will be done using the Statistical Package for the Social Sciences (SPSS v.20) software. Descriptive variables will be reported using percentages and charts, while continuous variables will be reported as mean and standard deviation. A p-value of <0.05 will be considered statistically significant. Any other statistical tools found to be appropriate for the study will be added during the time of study.
