2. OPEN THE COUNSELING WEBSITE
hpmds.smartpad.in/

3. After opening the Counseling Portal
Click on
“ONLINE APPLICATION” tab.
In case you have already filled
the “ONLINE APPLICATION”
Form. Click on “LOGIN” and
complete the remaining steps.

4. Fill the Online Application Form, then click on I agree to the terms &
conditions & click on Submit button.

5. DOCUMENTS UPLOAD
To view the uploaded document
Click on icon .
To Delete any
document that is
already uploaded
click on
button
Select the document to be uploaded>choose file to be uploaded
>Click upload button to upload the document.

6. After Uploading the required documents on the documents upload page
Click on the check box ‘I certify that all information is correct to the best of
my knowledge’ an then click on ‘ONLINE PAYMENT’ button.

7. ONLINE PAYMENT
Kindly pay the fees online by any of the mentioned payment methods.

8. CHOICE FORM
Fill the choice form as shown below.

10. PRINT FORM
After filling the choice form you can download the print form from the
Print form button as shown below

11. EDIT APPLICATION
In case you want to edit your application you can do this by clicking on
“Edit Application” tab on the left hand side.
