Ward round participition
Ward rounds are structured visits by a multidiscplinary healthcare team ( doctors,nurses,pharmacists,physiotherapists,dieticians etc.) to patients in a hospital ward. The goal is to review each patient’s condition,therapy and progress, and make care decisions. For a clinical pharmacists, ward round participition is a essential part of pharmaceutical care and clinical pharmacy practice. Introduction
Preparation before ward rounds :- -Review patient charts ( medications,renal functions,allergies,lab reports). -Identify potential problems (drug interaction,inappropriate dosages,unecessary medications,duplication ). -Check treatment guidelines (confirms that ongoing therapy matches hospital/national guidelines). Role of clinical pharmacists in ward rounds
Active participition during ward round :- -Medication review (drug choice,dose,route,frequency ). - Drug information provider (providing information about drug indication,mechanism,adverse effects,monitoring,alternatives to doctors/nurses). -Identifying and preventing medication errors (detect wrong doses,duplications,contraindicated drugs and preventig ADRs).
