DEFINATION Warfarin  is an  anticoagulant  used as a  medication  under several brand names including Coumadin. While the drug is described as a "blood thinner", it does not reduce viscosity but rather inhibits coagulation. BROAD CLASSIFICATION

SUB CLASSIFICATION ANTICOAGULANT has three sub- classes :- Heparins and low molecular weight heparins [ eg.dalteparin ] Caomadin derivatives [ eg . Warfarin ] Indandione derivatives [ eg . Phenindione,Anisindione ]

INDICATION Artrial Fibrillation or flutter Post myocardial infarction Mechanical heart valve Coronary heart disease Joint replacement Pulmonary embolism Venous thrombosis Stroke

Dosages and Routes Adult dose Venous thrombosis – 2mg -5 mg [IV or oral] For 2 days ; once a day. Post myocardial infarction – 2 – 5 mg[Oral or IV] for 2 days ; once a day . Stroke or Thromboembolism - 2 – 5 mg[Oral or IV] for 2 days ; once a day.

ADVERSE EFFECTS W - Wound healing delayed A - Alopecia R - Rashes F - Fetal warfarin syndrome[if used during pregnancy] A - Anorexia R – Rrqires monitoring of INR I - Interoots with many drugs and foods N - Necrosis ,skin [early adverse effect]

CONTRAINDICATIONS Preganancy Bleeding tendencies Hemorrhagic tendencies or blood dyscrasias . Threatned abortion , eclampsia , pre- eclampsia Malignant hypertension Major regional or lumbar block anesthesia

ANTIDOTE Vitamin K 1  is the only effective antidote for long-term management, but it takes several hours to reverse anticoagulation. Oral vitamin K 1  has excellent bioavailability, is rapidly absorbed, and is recommended in the absence of serious or life-threatening hemorrhage .
