Epidemiological Triad Agent ( B. Pertussis ) Host(infants Environmental &preschool childre n , (over crowding ,winter & below age of 5 years spring season)

Agent The causative agent of whooping cough is B. pertussis . The agent is present in the nasopharyrngeal , bronchial secretions of a case of whooping cough. Objective which are freshly contaminated are also the source of infection as there are infective . Whooping cough is most infectious during catarrhal stage. The period of communicability is considered as one week after the exposure to about 3 weeks after the onset of paroxysmal stage.

Host factors Whooping cough is mostly occurs in infants and pre – school children. Its high incidence is seen in children below 5 years of age. The incidence and mortality is more in males than females.

Environmental factors It occurs in overcrowding areas. More cases occur during the winter and spring months . Low socio-economic is also one of the factor of environment which is responsible for the whooping cough .

Prevention & control of whooping cough Early Diagnosis : The whooping cough should be diagnosed as early as possible .It is made possible by bacteriological examination of nasopharyngeal secretions taken from nose & throat. Isolation : The patient should be isolated until considered to be non – infectious. Treatment : The choice of treatment is erythromycin for the cases. Erythromycin : 30 – 50 mg/kg of body weight in divided 4 doses X 10 days other drugs which can be used for treatment purpose is ampicillin , septran or tetracycline. Prophylactically erythromycin or ampicillin is given for 10 days to those who come in contact with cases. This is given to prevent the establishment of infectious bacteria in the body. Immunization : A) Combined vaccine : DPT is administered intra muscularly in three doses at an interval of 1 month. It is started , when the age of infant is 1 ½ months. Then a booster dose of DPT is administered at age of 18 – 24 months. This combined vaccine protects the infants against diphtheria, whooping cough and tetanus. B) Single vaccine : Single vaccine is killed whole cell preparation and is effective against pertussis . The dose of conventional vaccine is 0.5 ml. It is given by I/m or deep S/C injection and is given in 3 doses, at an interval of 6 to 8 weeks, starting when the infants is 2 to 3 months old. Two booster doses at an interval of 3 years are given to step up immunity. Note : Local reaction at injection site , mild fever & irritability can occur due to pertussis vaccine. It should not be given to individuals with epilepsy , convulsions , febrile conditions, allergy to triple vaccine DPT or any person having CNS disorder.
