SUICIDE GENERAL CHARACTERISTICS : Suicide is never a random act Whether committed impulsively or after painstaking consideration, the act has both a message and a purpose Suicide is a means of escape

SELF DESTRUCTIVE BEHAVIOR Is an action by which a person emotionally, socially, and physically damages or ends his life. Typical self destructive behaviors : Biting one’s nails pulling one’s hair scratching or cutting one’s wrist

SELF DESTRUCTIVE BEHAVIOR COMPLETED SUICIDE : - IS THE MOST VIOLENT SELF DESTRUCTIVE BEHAVIOR.

LEVELS OF SELF DESTRUCTIVE BEHAVIOR Suicidal threat - threat more serious than a casual statement of suicidal intent and accompanied by behavioral changes : Examples : Mood swings Temper outbursts Decline in school or work performance

LEVELS OF SELF DESTRUCTIVE BEHAVIOR 2. Suicidal gesture – is a more serious warning signal than a threat that may be followed by a suicidal act that is carefully planned to attract attention, without seriously injuring himself/herself.

LEVELS OF SELF DESTRUCTIVE BEHAVIOR 3. Suicidal attempt – is a strong and desperate call for help involving a definite risk

Recognizing suicide potential - Major depression is an increased risk for suicide. - As the mood begins to elevate and depressive feelings lift , patient is considered to be at an even higher risk for suicide

QUICK ASSESSMENT 1. The nature of the problem. 2. The severity / seriousness of the problem. 3. The latest stressor. 4. The multiple stressors. 5. Suicide THOUGHTS / IDEATION . Present? If positive, go further. Determine whether PLAN is accessible

HIGH RISK FACTORS 1 . ACCESSIBILITY / AVAILABILITY OF THE PLAN = HIGH RISK Some preparations Weapon / agent already available? In the possession of the person? "Where is it now? the TIME, the DATE, the PLACE the AGENT or WEAPON to use, etc

2. PLAN includes the • NATURE •METHOD •AGENT or WEAPON to carry out the attempt or plan. • Use of a gun vs overdose. • Deeper wound(s) vs superficial injuries. • Sea-drowning for swimmer vs non- swimmer.

3. AMOUNT or TOXICITY OF the SUBSTANCE - HOW DANGEROUS POTENCY- Malathion vs chlorox 5 cups of chlorox vs 2 spoons of it Knife is less deadly than a gun Knife injury in belly or heart is deadlier than in the finger 20 tablets or capsules is deadlier than just 4 tablets 4 tablets of 1,000 mg. (4,000 mg. total) is deadlier than 20 tablets of 5 mg (100 mg. total).

4 . The ACCESSIBILITY to RESCUE or PROBABILITY for DISCOVERY Is the place / location frequented by people? Is it close to medical help? Is it isolated or visible to others? At the time of the act, is someone expected to come? The TIME (at night or broad daylight?). (But don't forget, suicide takes place when least expected).

5. The KNOWLEDGE of the DEGREE OF RISK 6. SUICIDE HISTORY / NO. of ATTEMPTS-- indicates higher risk than the first attempter 7. The FEELING of being "RELIEVED"

NOTE : Suicide takes place when least expected. Most do it when they are still going into the stage of depression or emerging from it. Thus, they have the necessary energy, concentration and volition to kill themselves.

NURSING INTERVENTIONS : Any threat must always be taken seriously Talk about suicide openly and directly Implement basic suicide precautions Check client every 15 minutes stay with client while giving meds Search belongings check articles brought by visitors

NURSING INTERVENTIONS : Maximum suicide precaution Non judgmental caring attitude Ask why attempt to commit suicide Make a “ NO suicide contract “ Do not make unrealistic promises Evaluate client needs for medication Do body examination of the client
