2. ACCOMPLISHMENT REPORT AS OF SEPTEMBER 2024 GENERAL FUNCTION : To be responsible for the Civil Registration program in the Local Government Unit pursuant to Civil Registry Law, the Family Code of the Philippines & other pertinent laws, rules and regulations issued.    Civil Registration is a vital aspect of our life. It is our duty to register life’s circumstances from birth, marriage and death. In whatever form of society we belong, it is our fundamental right to be registered and counted.

3. SPECIFIC FUNCTION/DUTIES: A. Received & Recorded Civil Registry documents from January -December 2024 LEGAL INSTRUMENTS such as: Registered Vital Events Total Birth 1,223 Marriage 39 Death 277 Total Affidavit to Use the Surname of the Father 584 Affidavit of Legitimation 24 Affidavit of Admission of Paternity 10 Court Decrees/Orders 14

6. PHILSYS STEP 2 REGISTRATION OUTPUT MONITORING AS OF SEPTEMBER 5,2024   Total Target Registrants (Included 1-4 & 5 yrs. & up yrs. BRAP (Barangay Registration Assistance Project)   In connection with the continuous implementation of the Birth Registration Assistance Project (BRAP) we conducted last September 12,2024 a Mobile Registration to Barangay Burgos & Sta. 7. Awarding of SECPA to the first batch of registrants registered under BRAP. The PSA, Provincial Office was represented by Kenneth Ivander R. Alivio.

8. On BREQS (Batch Request Query System)   Request for PSA copy of Birth, Marriage, Death & CENOMAR thru BREQS is increasing. As of September 2024, we have a total of 442 released PSA copies.
