Cultural Anthropological, Sociological, and Socio-Psychological Perspectives on Dengue: A Comprehensive Approach to Planning, Implementation, Evaluation, and Assessment(PIME) Presented By: Nabin Parajuli Roshan Thapa MPH 2 nd semester Pokhara University, Kaski, Nepal

Traits of Cultural Anthropology

Conceptual framework. Favourable Unfavourable Modern Culture Tradition Taboos Belief system Rituals Construction of HBM model for intervention Using insect repellents and not adopting sustainable practices   Isolation to infect Social media spreading fake news   Papaya leaf, goat milk, crab soup Having infected Neighbour Social Ties, Social proximities, gatherings Herbal plants for treatment and repellent Person to person transmission, strong body   Drone Sample collection and mapping, Mobile app Not using clothes of dead peoples

Tradition Favourable and unfavourable favourable Euphoria Hirta is used for treatment of DF as it has broad medicinal properties, including Anti-malaria, Antiviral , Anti-inflammatory, Anti bacterial properties.- (Kumar et al., 2010) ( Perera et al., 2018) Medicinal herbal plants like Mayana , Tawa-tawa to cure dengue and malaria.- ( Gascon , 2011 ) Methanol extracts of  Delonix elata  (L.) Gamble leaves and seed offered protection for over 180 and 150 min, respectively, against  Ae . Aegypti . ( Govindarajan et al., 2013) Unfavourable Cultural perceptions of dengue fever can impact health care seeking behaviour ( Rana , n.d . ) movement patterns are structured by social ties , and social proximity may be a good predictor of infection risk for DENV and other pathogens transmitted by the mosquito  Aedes aegypti ( Zellweger et al., 2017 )

Modern culture Favourable and unfavourable Favourable Behavioural factors, Demographic factors, Economic factors are important for effective mitigation of dengue, Construction of HBM are useful for developing intervention for prevention and control of dengue. (Wong et al., 2014 ) The NepaDengue application has been perceived as a promising tool for dengue prevention and control in Nepal ( Mahotra et al., 2024) A drone inspection system allows for the collection and analysis of water samples, facilitating the creation of dengue transmission maps (Tsai et al., 2024 ) Unfavourable Believed that insect repellents or fly swatters were effective in controlling mosquito populations not adopting sustainable practices (“8 myths about dengue debunked,” n.d . ) In Brazil, for instance, the circulation of fake news during a dengue prevention study created resistance among communities, complicating efforts to implement effective health interventions (Gagnon- Dufresne et al., 2023 )

Taboos Favourable and unfavourable Favourable Isolating a person with dengue fever may reduce the number of mosquitoes that can be infected (Arellano et al., 2015a) Unfavourable In some dengue endemic countries people use remedies such as papaya leaf juice/extract, crab soup and to treat dengue ( Ismail et al., 2019)

Belief system Favourable and unfavourable Favourable lack of preventive measures from the community level and having a neighbour with dengue can lid to dengue causation (Wong et al., 2014) . Unfavourable some individuals believe that dengue can spread from person to person , similar to influenza (Wong and AbuBakar , 2013) . Majority of young male, who were confident that they have strong body defences against dengue and will not contract dengue (Wong and AbuBakar , 2013 ) People did not seek timely dengue treatment because they did not know the symptoms of dengue and classified dengue as the flu , common cold or fever (“Factors influencing healthcare seeking in patients with dengue: Systematic review - Ng - 2022 - Tropical Medicine & International Health - Wiley Online Library,” 2022)

Rituals Favourable and Unfavourable Unfavourable Study revealed that community members perceived, transmission to occur even through contact with inanimate objects , known as fomites. For example, practices undertaken when a person dies from dengue, such as throwing away the bed of the person and their clothes, are related to a social representation of dengue as a disease caused by a miasma (Arellano et al., 2015b) .

Traits of Sociology

Favourable Effect of S ociological Trait Ethnicity: Black individuals are at lower risk for the most severe dengue clinical course compared with white individuals (Sierra et al., 2006) . Family Size: Householders who have ≤ 4 family members are 1.74 times more likely to have adequate health seeking behaviour and 1.85 times more likely to have adequate Dengue Prevention Behaviour ( Rajapaksha et al., 2023) .

Contd.. Socio-economic: A higher percentage of big apartments was associated with lower dengue incidence rates during epidemic ( Zellweger et al., 2017 ). Compared to high strata neighbourhoods, low and middle strata neighbourhoods had significantly less knowledge of DENF, CHIK, and Zika ( Desjardins et al., 2020 ). Legislative Process: The legislative Thailand momentum on policy and practices decision involves the assignment of public health officials by the Minister of Public Health to implement dengue vector control measures ( Bhumiratana et al., 2014).

Contd.. Executive Process: Advisory Committee on Immunization Practices (ACIP) recommend for use of the Dengvaxia vaccine in the United States (Paz-Bailey et al., 2021 ). 10 minute sanitation program every Friday around health facility as per ruling of MOHP. Philippines government adopted environmental control measures, intensified chemical fogging, and reminded its citizens to follow the “ 4 o’clock habit ” ( Dhoubhadel et al., 2023 )

Unfavourable Effect of Sociological Trait Socio-economic some studies suggest that improvements such as better sanitation systems and higher GDP may not always correlate with lower dengue prevalence but sustainable practices and high temperature can support causation (Simon and Rangel, 2021 ) Lower socioeconomic status and higher poverty levels are associated with a higher incidence of  Aedes -borne diseases ( Zellweger et al., 2017) Evidence from systematic review suggests that lower SEP increases the risk of acquiring arboviral infection (Power et al., 2022 ) There is a significant association between respondent's dengue history and houses that are shaded with vegetation (p= 0.012) and the present of public playground areas near the residential (p = 0.011) (Adnan et al., 2021) .

Contd.. Political Decaying public health infrastructure, due to political neglect or mismanagement, hinders effective dengue surveillance, prevention, and control ( Gubler , 2011) Political and economic crises leading to mass migration can strain public health systems and exacerbate dengue transmission, as seen in the Venezuela-Colombia border region- (Morgan et al., 2021)

Contd.. Cast system CHIKV and dengue is associated with scheduled and forward castes, households with improper toilet facilities, open defecation, and kutcha house type (Paulson et al., 2022)

Traits of Socio-Psychology

Conceptual framework

Group cognition Favourable and Unfavourable Favourable Unfavourable false beliefs/ misinformation about mosquito breeding habits or treatment methods can hinder prevention efforts. significant associations were found between practices for control and community shared information during dengue outbreak( Zahir , 2016 ). Behavioral change towards attaining sustainability in dengue preventive practices may be enhanced by fostering comprehensive knowledge of dengue and a change in health beliefs(Wong and AbuBakar , 2013b ). false beliefs/ misinformation about mosquito breeding habits or treatment methods can hinder prevention ( Mejía et al., 2016) )

Group motivation Favourable and Unfavourable Favourable Unfavourable Motivation fosters a sense of ownership in the community, encouraging active participation in developing and sustaining dengue prevention plans (sense of ownership)( Arfan et al., 2024) C ertain community members are excluded from decision-making processes, face resources constrain and support from local government it can decrease their motivation to participate. (‘(PDF) Community Participation in Mosquito Breeding Site Control’, 2024)

Group dynamics: Favourable and unfavourable Favourable Unfavourable Effective communication, including clear and aesthetically pleasing messages, motivates individuals to take appropriate actions( Samsudin et al., 2024) Poor coordination among different stakeholders, such as community groups, government agencies, and NGOs, can lead to fragmented and ineffective dengue control effort. (Nguyen- Tien et al., 2019)

Group identity Favourable and unfavourable Favourable This study revealed that having history of dengue infection are barriers and facilitators influencing healthcare seeking Unfavourable Trusted community leaders and members can effectively communicate important messages and encourage preventive behaviours ( Samsudin et al., 2024 ). Marginalized groups may be less likely to participate in dengue prevention efforts or seek medical care when needed ( Samsudin et al., 2024 ).

PIME PIME Description Planning Identify various factors affecting dengue health seeking behaviour.(individual, organizational and community) (Perceiving dengue knowledge and non- evidence based Traditional and complementary medicine use via social media) ( multivitamins 51%, crab soup 85%, papaya extract 65%) (Hwang et al., 2020) Post graduates 25%, graduates43% (exploratory study,2024) Implementation Social media's ubiquitous presence in our everyday life, health professionals should use these tools, helping users improve their health literacy, health- seeking behaviour. Use of mobile apps (Mo-Buzz, Nepadengue that integrates – surveillance, mapping and education) ( Lwin et al., 2016) ( Mahotra et al., 2024) Monitoring and evaluation Monitoring the prescription of multivitamins and media advertise of pharmaceutical companies. Conduct KAP studies and track the outcome indicators.

Traits linked in public health strategies P3CE Strategies Promotion Utilize social media platforms to disseminate accurate, evidence-based information debunking myths. Conduct school-based programs to educate children and families about dengue prevention . Prevention Implement Integrated Vector Management (IVM) strategies, public awareness campaigns through digital tools. Educate sustainable preventive practices. Organize sensitization meetings in health facility, school and community. Protection Early Notification system to general public that support adopting preventive practices. use of “endemic channels” as a tool for early detection of dengue outbreaks. Control Strengthen health system capacity Integrate school, health facility and stakeholders Control dissemination of misinformation to public Legislation for multivitamins prescriptions and promotion Encouragement Promote innovative technologies that disseminate evidence based findings. Recognize and reward communities or individuals who actively participate in dengue control efforts. Encourage community people to follow the authentic source to gain health literacy
