INTRODUCTION Cardiac glycoside are a class of organic compounds that increases the output force of heart and decrease the rate of contractions by acting on the cellular Na+ / k+ -ATPase pump.
Sources of Cardiac glycoside NAMES Digoxin Digitoxin Bufotoxin Ouabain OBTAINS FROM Digitalis lanata Digitalis purpurea Toad skin Strophanthus gratus
PHARMACOLOGYCAL ACTION 1. Heart contraction 2. Blood vessels Digitalis directly effects on myocardial contractility, so it decreases Digitalis has mild direct vasoconstrictor action . It has no prominent effect on blood pressure but , for CHF patients ,it increases actinosystolic B.P.
3. CNS 4. Kidney It shows little effect on CNS normally, but if consumes higher dose of cardiac glycoside , then it activates CTZ (chemoreceptor trigger zone). Diuresis occurs for CHF patients but dose not occurs for normal patients though.
MECHANISM OF ACTION
ADVERSE EFFECT 1) CARDIAC almost every types of arrhythmia can be produce by digitalis. Examples____ Ventricular arrhythmias Lidocain Supraventricular arrhythmias Propanolo A-v block & Bradycardia Atropine
uses Digitalis is used in the treatment of congestive heart failure ( systolic & diastolic dysfunctions ). Digitalis most important drug for different cardiac arrhythmias . Digitalis is use for controlling ventricular rate in atrial filtration
INTERACTIONS Diuretics Quinidine Calcium Adrenergic drugs Others It increases the risk of digitalis arrhythmias. It reduces the binding of digoxin to tissue proteins. These are synergises with digitalis and precipitate toxicity. These drugs are induce arrhythmias. Verapamil , Captopril increases plasma concentration of digoxin.
