A 18 month old female child was brought by the parents to Community Health Centre in Moodbidri with complains of breathlessness and fever since 3 days . What are the differential diagnosis based on the presenting complaints? What other important history would be elicited in arriving at the diagnosis for the present case?
On obtaining detailed history it was noted that the child was apparently normal few days back when it developed breathlessness associated with cough. It was sudden in onset and progressive in nature. It also developed fever which was mild and intermittent. No history of chest or abdominal pain No history of difficulty swallowing No history of excessive drowsiness No history of asthma
What was the significance in asking drowsiness or difficulty in consumption of food?
Examination Child was conscious, alert and cooperative and well oriented to time place and person. The child was afebrile with a pulse rate of 98/minute and respiratory rate of 44 per minute. Chest in drawing was noted on inspection when the child breaths in. Resonant note was heard on percussion and no signs of wheeze was noted on auscultation.

Treatment How do you manage this case and the protocol to be followed in treating the present case? Child was treated with benzyl pencillin and was monitored closely and reviewed after 48 hours of antibiotic therapy
