Cervical Spondylosis It is degenerative condition of the cervical spine Age – most common in person over 50 yrs of age More in people – white collar job Neck strain Neck in constant one position while reading and writing Common site – lowest 3 cervical IVJ (C5 – C6)

Pathology Starts in the intervertebral disc ↓ Degeneration of disc occurs ↓ Reduction of disc space ↓ Formation of peripheral osteophyte ↓ 1. Neck pain – due to involvement of post. intervertebral joint 2. May present as sign of cord compression – because of osteophytes pressing on spinal cord 3. Radicular pain in upper limb due to osteophytes impinging on nerve roots

Clinical Features Pain and stiffness – initially → intermittent latter → persistent Radiating pain – to shoulder / downward on outer aspect of forearm and hand Paresthesia – in region of nerve root commonly over base of thumb ( along C6 nerve root ) Giddiness – episodes because of vertebro – basilar syndrome Occipital headache – if upper half of cervical spine is affected

Examination Loss of normal cervical lordosis Limitation of neck movement Tenderness over lower cervical spine Tenderness in the muscle of Para – vertebral region ( Myalgia ) Muscle weakness in uncommon → Examine the lower limbs for any signs of early cord compression

Radiological Findings X – ray cervical spine → AP and Lateral view Shows – 1. Narrowing of IVD space ( most common C5 – C6 ) 2. Osteophytes at vertebral margins presenting at anteriorly and posteriorly

Differential Diagnosis Other causes of neck pain – Infection Tumours Cervical disc prolapse Other cause of upper limb pain – Pancoast tumour Cervical rib Spinal cord tumour

Treatment Aim – to assist the natural resolution of temporarily inflamed soft tissue During period of remission, prevention of further attack is utmost Advice to patient → Proper neck posture – not to keep his neck in one position for long time Neck muscle exercise – help in improving neck posture

Treatment in episode of exacerbation – Analgesic Hot fomentation Rest neck in the cervical collar In neck stiffness → traction given If giddiness present → Anti - emetics given
