INTRODUCTION Myself Mahesh M S , HSES Training Specialist of McDermott ( CB&I) Pvt Ltd working since July 2021 ,have experience in UAE ,KSA , KUWAIT & INDIA Welcome you all for Confine space Training Session . I understood you all are involved in Tank work and have more than 4 years experience . This will be a refresher training and remind er for roles & responsibilities of each person. CONFINED SPACE Purpose The purpose of this module is to teach participants the requirements for Confined Space Entry and explain the duties and responsibilities of employees acting as Supervisors. Attendants and Entrants of a confined space entry. This course will be a review of the correct safety tools that are part of our HSE management system requirements. Additionally, this course will be used to: R e-fresh supervisor and employee knowledge of the Confined Space procedure. Require supervisors and employees to review their current approach to HSE implementation Challenge supervisors and employees to continuously improve HSE performance within their areas of responsibility Establish and define CB&I HSE expectations Provide supervisors and employees with the Systems and Processes to effect change and achieve our HSE goals Provide an HSE leadership education process for all leaders.
COURSE OBJECTIVES
CONFINED SPACE ‘A confined space is a place which is substantially, though not always entirely, enclosed and where serious injury can occur from hazardous substances or conditions within the space or nearby’ Another way to think about what is a Confined Space is to ask if….. Is the space large enough to enter and work? Does the space have limited openings for Entry and Exit? Is not designed for Continuous Worker Occupancy

JOB PLANNING Can the task be done without the need for personnel to enter the confined space? Is the work really necessary, or could you: Modify the confined space itself so that entry is not necessary Have the work done from outside Use equipment and tools to avoid entry Use online Inspection or remote cameras for internal inspection Use long-handled tools
RISK ASSESSMENT A risk assessment must be performed for all confined space entry work IDENTIFY ASSESS CONTROL RECOVER Are people, environment or assets exposed to potential harm? What can go wrong? What are the hazards? What can cause it to go wrong? What is the risk? How likely is it to occur? How bad will it be? Can the causes be eliminated? How can it be prevented? How effective are the controls? Can the potential consequences be limited? What recovery measures are needed? Are recovery capabilities suitable and sufficient?
CSE PERMIT If confined space entry is unavoidable, then an approved Confined Space Permit, under the Permit to Work system, must be obtained in order to proceed with work. The confined space permit is separate from any other permits and must be prepared with input from personnel responsible for the overall work on site during the entry and those who will be leading the entry work .

ROLES & RESPONSIBILITIES Site Engineer is responsible for the identification of all Confined Spaces must ensure that all personnel involved in Confined Space operations are competent and comply with the company standards and PTW system Entry Supervisor is responsible for the safe access and egress of entrants understands all the hazards and communicate these to all personnel understand the signs and symptoms of exposure and communicate these to all personnel must ensure all tests, equipment requirements, personnel and procedures specified on the Confined Entry Certificate are verified must stop entry and cancel the permit if the conditions of the permit and certificate are not met
ROLES & RESPONSIBILITIES Confined Space Entrant must follow requirements of the Permit and Confined Space Entry Certificate must know and recognize the hazards that may be faced during the confined space activities and be able to recognize potentially hazardous changes in conditions should alert the Confined Space Attendant should an unsafe condition arise maintain communication with Attendant comply with orders of the Attendant ensure all tools are removed upon completion of work
ROLES & RESPONSIBILITIES Hole Watcher / Attendant must follow requirements of the Permit and Confined Space Entry Certificate must know and recognize the hazards that may be faced during the confined space activities and be able to recognize potentially hazardous changes in conditions should alert the Confined Space Attendant should an unsafe condition arise maintain communication with Attendant comply with orders of the Attendant ensure all tools are removed upon completion of work DON’T ENTER A CONFINED SPACE
ROLES & RESPONSIBILITIES Hole Watcher / Attendant Keep Entrant under surveillance and maintain communication Line of sight ( not always possible ) Voice contact Radio signals Whistles Pre arranged lifeline signals etc.. Summons assistance should an entrant get in trouble.
ROLES & RESPONSIBILITIES Authorized Gas Tester A person who has passed Authorized Gas Testing Training & approved & certified by project management . Performs tests before start of work & in between breaks Proves the oxygen is in safe level Proves & certifies the atmosphere inside the confined space is safe to work. Only to perform with calibrated instrument Should perform after isolation & in the presence of Supervisor & attendant Where practicably the test shall be performed from out side with extension probes.
GAS TESTING Air Quality Criteria Oxygen Content ( Volume ) 19.5 % – 23.5 % ( OSHA ) Flammables Less than 10% of the LEL where Hot work is to be done 0% Toxic Gases Carbon Monoxide – < 35 ppm Hydrogen sulfide - < 10 ppm
